📢 New publication: “A Framework for Sensemaking and Advancing the Continuum of Integrated Care: From Condition-Based Pathways to Population Health”

This article introduces a practical framework to help understand how integrated care evolves — from focused clinical pathways to whole-population approaches — and what this means for improving care, experience and outcomes.
