How do we turn integrated care policy into meaningful change on the ground?
This session explores how health and care systems can move beyond strategy into implementation, translating ambition into coordinated, person-centred practice. It will examine the realities of working across complex systems, including governance, workforce, culture and leadership.
Drawing on international experience, the discussion will highlight practical approaches to bridging policy and practice, supporting collaboration and sustaining change at scale.
