Information is gathered via assessments, visits and discussions with
clients, carers and relevant professionals.
-Assessment
A specialist comprehensive assessment is conducted including past and
current medical and psychiatric diagnosis and treatment, behavioural,
mood, cognitive, functional domains, risk assessments and
management strategies.
-Placement & Care Planning
an individualised person-centered multi-disciplinary care package
(with costing) - including staffing ratios, activities and resources is
available for approval to stake holders prior to placement.
- Review
Initial care plans are reviewed and then refined/amended during the
first three months of placement.
