These services may include:
✔️ Assessment of minor injuries
✔️ Wound care
✔️ Chronic disease monitoring
✔️ Blood testing
✔️ Ear care

When you contact your GP surgery, you may be offered an appointment with the Community Treatment and Care team instead.

📞 In Perth and Kinross, you might be asked to call 01738 413099. This number will connect you directly to the team, who will arrange an appointment at your local GP practice or a Community Treatment and Care Hub.
