Let's look at nutrition and wound healing:
Be honest, how often do you practitioners assess a patient with a non-healing wound and consider nutritional support? Evidence strongly suggests that it isn't happening very often - except in patients with diabetes or burns. So, where do we start? This is not a topic to be covered in one post so we will begin with an intro to nutritional screening and assessment. Firstly, make no assumptions: patients with a low BMI may appear malnourished, those with a high BMI are not necessarily well- or over-nourished. Adequate nutrition depends on many factors e.g. energy requirement, activity, general health status and clinical setting. As a starting point the MUST tool is good. Tetanus in patients with chronic wounds.
Have you considered the possibility that such patients might need a booster? or, that tetanus is an issue clinically? Well the answer is a definite "Yes" to this latter question. A recent bulletin on wound healing from the eminent Dr Fife in America reminds us of the need, and the dangers for this patient group. Included are all patients with leg ulcers, pressure ulcers and diabetic foot ulcers.
Dr Fife reports that the ACIP in America is recommending vaccinations against tetanus for high risk groups including persons who have wounds. The evidence supporting this comes, in part, from a Cardiff study by Professor Harding's group published in the International Wound Journal 9(1); 93-99. An article to follow up for sure. Antibiotic stewardship demands "appropriate" use. The management of UTIs in particular is rarely evidence-based but rather ad hoc trimethoprim, nitrofurantion until the infection will not resolve. Only then is an objective approach taken. Same story for cellulitis.
