🩺 Chronic Disease Self-Management
📅 Duration: 6 weeks
🕒 Sessions: 2.5 hours each
💬 Learn practical skills to manage your condition, improve your lifestyle, and connect with others who understand your journey.

💻 All sessions are delivered by trained tutors who have lived experience with long-term health conditions.
