Hi, I’m Josh Devine - Physiotherapist, chronic pain specialist, and founder of BetterMe Physiotherapy in Medway.

Two sessions.
One completely different person.

When I first met this patient, she walked in carrying 20 years of fear.

Chronic lower back pain had slowly taken pieces of her life away,

exercise stopped, confidence disappeared,
everyday movement became something to avoid rather than enjoy.

She’d once been told her back was “out of position,” and from that moment on, she lived believing she was fragile… that one wrong move could cause damage.

So she stopped moving.
Stopped trusting her body.
Stopped being herself.

Instead of chasing pain or trying to “fix” something that wasn’t broken, we changed the story.

Most people don’t come to physio because of pain.

They come because of what pain has slowly taken away.

Recently, someone said something that really stayed with me:

“I feel like a burden to my family… I’m always the one saying I can’t do that activity. I feel like I slow everyone down.”

Then they added,
“I don’t really mention the pain anymore. I don’t feel believed… people say I look fine.”

And that’s the part of persistent pain many people carry quietly.

From the outside, life looks normal.
But inside, every plan comes with hesitation-

how far you can walk, whether it will flare up, whether you’ll have to sit things out again.

Over time, people stop talking about how they feel.

They push through.
They adapt.

This client had hurt their back before and normally it settled within a few days.
But this time… it didn’t.

A week later they were still in agony, frustrated, and unsure what to do next.

Physio wasn’t something they’d tried before, but they decided to give it a chance after finding my page.

What mattered most wasn’t just treating the painful spot.

It was stepping back, looking at the whole picture, and helping them understand why their body wasn’t settling this time.

After just one session, their pain eased and movement started to feel possible again.

“I tried physio before… and it didn’t work.”

It’s something I hear often.
And if that’s been your experience, it makes complete sense to feel hesitant about trying again.

But more often than not, the issue isn’t that physiotherapy failed.

It’s that the treatment was aimed at the pain rather than the person experiencing it.

Pain rarely exists in isolation.

It’s influenced by how you move, how you load your body, your training history, your stress levels, your recovery, even your sleep.

When these pieces are overlooked, treatment can become short-term relief instead of lasting change.

My approach is different.

I look at the bigger picture, because real recovery usually requires more than a quick fix or a sheet of exercises.

It requires understanding why the problem developed in the first place, and what needs to change so it doesn’t keep returning.

You are not just a knee, a shoulder, or a back.
And you shouldn’t be treated like one.

“Your pain isn’t what you think it is.”

Recently, I met a man who had stopped playing 5-a-side football.

Not because he physically couldn’t.

Because he was afraid to.

He had been told his back pain was caused by disc damage.

So he did what most people would do, he protected himself.

He moved less.
Avoided sport.
Became cautious in everyday life.

All because he believed pain meant harm.

But when I assessed him, something didn’t add up.

✔ Full range of movement
✔ No spinal pain during testing
✔ No neurological symptoms
✔ Strong and capable

His body wasn’t the problem.

Fear was.

At one point I asked him:

“What would it mean for you to play football again?”

He didn’t hesitate.

“Everything.”

That answer stuck with me.

Because this is the hidden cost of misunderstood pain.

Not just discomfort…

But lost identity.
Lost confidence.
Lost joy.

Words matter.

Especially in healthcare.

When pain is explained purely through scans and structures, people can unknowingly begin to see their bodies as fragile.

But pain is rarely that simple.

It is influenced by the nervous system, past experiences, stress, sleep, beliefs, and perceived threat.

And here’s what I want you to hear if you’ve ever been told something similar:

Pain does NOT automatically equal damage.

Bodies are adaptable.
Spines are strong.
And the right guidance can rebuild trust in your body.

The goal is never to ignore pain.

The goal is to understand it so it stops controlling your life.

No one should have to give up the things that make them feel alive because of fear.

Understanding pain is freedom.

Because when you change the story around your pain…

You change what feels possible again.
