We are looking for a : SUPPORTED LIVING CARE CO-ORDINATOR.
JOB DESCRITION:
We are looking for an enthusiastic, creative and committed Supported Living Care Co-ordinator to join our Reading team in providing high quality support and care services to adults with people with learning disabilities, autism, mental health and other physical health care needs.
We provide 24-hour care to individuals in 3 separate services in Reading & West Berkshire Purley On Thames.
As a Supported Living care coordinator your duties will include:
• Managing the staff / supporting the individuals in their properties on a day-to-day basis including supervisions and staff rotas.
• Coordinating care and support for individuals and supporting individuals with their reviews and care needs for long term care and support.
• Demonstrate good IT skills and be able to record information clearly, precisely, accurately, in a timely manner and professionally.
• Liaise professionally with a range of external stakeholders, care managers, health professionals, other services and providers of support, and family members
• Delivering a service that promotes and supports choice, empowers individuals, helps people take control of their own support and care, and increases their independence and self-determination.
• Ensuring the service meets regulatory requirements.
• Promote a professional image of the service through excellent communication, appropriate behaviour, professional boundaries and professional appearance.
• Ensure that the management of medication is safely administered in the services including the recording.
• Develop support plans based on individual service users' assessed needs and choices.
• Report incidents promptly and ensure appropriate action and learning are incorporated into care delivery.
• To provide staff cover in an emergency and on-call.
• Report any safeguarding concerns in a timely manner and as per company policy and procedures.
• Maintain effective and clearly documented handover systems.
• Maintain high standards of record keeping in accordance with relevant legislation and company policies.
Essential Skills and Knowledge
• Must have previous experience in Supported Living Care.
• Must hold a clean UK driver’s licence.
• NVQ Level 3 or working towards NVQ level 4.
• Must have strong communication skills both written and verbal.
• English and Math GCSE Grade C or better Or equivalent

Salary: £24 K - £26.250 / Annum depending on experience
