📢Total Triage System Launching Monday 4th August📢

From Monday 4th August 2025, we are moving to a Total Triage system.

🗓️ Routine appointments are currently limited whilst we move over to the new system.

💻 For all routine requests, please complete a triage form during the week commencing Monday 4th August 2025.

📞 Before the change over date if you have an urgent on the day concern, please call the practice as usual.

Thank you for your patience and support as we make this change to improve access and manage demand more effectively.

Dear Patient,

To improve access and reduce wait times, we’re introducing a GP-led Total Triage system from Monday 4th August 2025.

From this date, all GP appointment and query requests (urgent and routine) will be reviewed by a GP on the same day. Based on your clinical needs, you’ll be offered the most appropriate outcome, such as an appointment, advice, or referral.

How to request help:
•Use our new online triage form on Theale Medical Centre – NHS GP Surgery in Theale, Reading, available Mon–Fri, 07:30am–4:00pm.
•Do not call for GP appointments – use the form instead unless urgent after 4:00pm.
•If you don’t have internet access, please call or visit reception, and we’ll complete the form for you.

You may be:
•Given an urgent or routine appointment
•Sent advice via phone, text, or email
•Referred to a nurse, pharmacist, social prescriber, or other services
•Directed to 111, pharmacy, or other local services

Nurse and healthcare assistant appointments remain bookable by phone.

If you have a life-threatening issue, call 999. For urgent concerns after 4pm, call the surgery.

Routine GP bookings will pause as we prepare to switch systems.

We appreciate your patience as we make this change to improve care and ensure timely access.

🌟 Mounjaro (Tirzepatide) – Important Information 🌟

We are aware of recent media reports suggesting that Mounjaro (Tirzepatide) is now available from GPs for weight loss.

Unfortunately, this information is somewhat misleading. While Mounjaro is now licensed and available under specific circumstances, only a very small number of patients currently meet the strict eligibility criteria.

👉 Please do not contact the surgery to request this medication.
If you are eligible, we will proactively contact you in the coming weeks to discuss next steps.

Any vehicles left in the car park after this time will be locked in and unable to exit until the following day.

If you are attending the fair, please do not use the car park, as it is only for patient use.

🏥 Bank Holiday Closure Notice 🏥

Please note that Theale Medical Centre will be closed on Monday 26th May 2025 for the Bank Holiday.

During this time, the practice will not be open for appointments, prescription requests, or general enquiries.

In a medical emergency, please call 999. For urgent medical advice that is not life-threatening, please call NHS 111, available 24/7.

Normal opening hours will resume on Tuesday 27th May 2025.
