Dear Patient,
To improve access and reduce wait times, we’re introducing a GP-led Total Triage system from Monday 4th August 2025.
From this date, all GP appointment and query requests (urgent and routine) will be reviewed by a GP on the same day. Based on your clinical needs, you’ll be offered the most appropriate outcome, such as an appointment, advice, or referral.
How to request help:
•Use our new online triage form on Theale Medical Centre – NHS GP Surgery in Theale, Reading, available Mon–Fri, 07:30am–4:00pm.
•Do not call for GP appointments – use the form instead unless urgent after 4:00pm.
•If you don’t have internet access, please call or visit reception, and we’ll complete the form for you.
You may be:
•Given an urgent or routine appointment
•Sent advice via phone, text, or email
•Referred to a nurse, pharmacist, social prescriber, or other services
•Directed to 111, pharmacy, or other local services
Nurse and healthcare assistant appointments remain bookable by phone.
If you have a life-threatening issue, call 999. For urgent concerns after 4pm, call the surgery.
Routine GP bookings will pause as we prepare to switch systems.
