Benign Positional Paroxysmal Vertigo (BPPV). BPPV is a condition that affects the inner ear, usually causing short-lived episodes of intense dizziness or vertigo (a sensation of room spin) when the head is moved in certain directions. It is often known as 'top shelf vertigo' since reaching up to a high shelf can trigger symptoms; but can also be triggered with turning over in bed, getting out of bed, reaching down to the floor to pick something up.

BPPV is thought to be caused by tiny crystals called otoconia being displaced into one of the semicircular canals of the inner ear. This can happen as a result of an inner ear infection, a head injury, migraine or sometimes just as a part of the ageing process.

How we assess and treat
We will firstly take a detailed history to help us understand the possible cause and the impact of the symptoms on your everyday activities. Thankfully, BPPV when correctly diagnosed, usually with a test called the Dix Hallpike, is very treatable with an Epley manoeuvre. This is a simple treatment involving moving your head into various positions over a few minutes. The technique uses gravity to move the tiny crystals away from where they are causing problems. In 80% of cases, one Epley manoeuvre can resolve symptoms completely.

If you are struggling with symptoms of dizziness, please call the clinic on 01634 817242 for an assessment with our vestibular physiotherapist, Julie Burge. We would love to be able to help.

Running injuries can affect anyone, from experienced runners who push themselves hard, to beginners whose muscles are not used to running. Dealing with knee pain that just won’t quit after a run? You might have runner’s knee (Patellofemoral pain syndrome).

What does runner's knee feel like?
During your run, you may develop pain at the front of the knee, around the knee, or behind the kneecap. The pain may be dull, or it could be sharp and severe.

Symptoms to watch for:
Dull, aching pain around or behind the kneecap
Pain when walking downstairs, squatting, or sitting for long periods
Clicking or grinding sensation in the knee

Why it happens:
Often due to overuse, muscle imbalances, poor running form or improper footwear.

How we assess and treat
We will firstly ask about your symptoms and activity levels to try and determine the mechanism of the injury. We will then carry out a full physical assessment to see which structures are impacted. This could also include gait analysis to assess if the way you are running is contributing to the pain.

Treatment options can include:
* Soft tissue massage to help relieve tension in affected muscles
* Joint mobilisations to help alleviate pain and stiffness
* Cryotherapy
* Myofascial release
* Acupuncture
* Taping
* Complex muscle stimulation
* Ultrasound

How physio helps:
- Targeted exercises to strengthen your hips & quads
- Gait and posture analysis
- Manual therapy & taping techniques
- Education to prevent recurrence

Whatever your injury, it's important to listen to your body. Do not run if you're in pain, and only start running again when you have recovered sufficiently. (Source: NHS website)

Whiplash is a neck injury caused by a sudden jerking motion of the head—typically forward and then backward—similar to the cracking of a whip. It is most commonly associated with rear-end car accidents but can also result from sports injuries, physical abuse, or falls. The rapid movement strains or sprains the soft tissues of the neck, such as:

- Muscles
- Ligaments
- Tendons
- Discs between the vertebrae

Symptoms often appear within 24 hours of the incident, but may be delayed by a few days and can include:

- Neck pain and stiffness
- Decreased range of motion in the neck
- Tenderness or pain in the shoulders, upper back, or arms
- Headaches, often starting at the base of the skull
- Tingling or numbness in the arms
- Dizziness
- Fatigue

How we assess and treat whiplash:

Our physiotherapist will carry out a detailed assessment to understand the mechanism of your injury, identify the areas affected and assess for any functional limitations. We will also ask about your lifestyle and hobbies to gain an understanding of how your symptoms are impacting your life.

Our goals are your goals. We aim to enable you to return to the activities you love through a tailored individualised programme of exercises and lifestyle advice to help you manage your symptoms, alongside appropriate treatments:

- Soft tissue massage to help relieve tension in affected muscles
- Joint mobilisations to help alleviate pain and stiffness
- Neural mobilisations to help with blood flow to affected nerves
- Relaxation techniques
- Myofascial release
- Acupuncture
- Taping

Dealing with shoulder pain that just won’t budge? It might be frozen shoulder (aka adhesive capsulitis). This condition causes stiffness, pain and a limited range of motion in the shoulder joint, making it tough to carry out everyday tasks like reaching, dressing or sleeping. It often develops gradually and can worsen over time.

Common symptoms include shoulder pain, especially at night, and a progressive loss of mobility. Treatment typically involves physical therapy to improve flexibility and strength, anti-inflammatory medications for pain relief, and in some cases, steroid injections or surgery if symptoms persist.

The good news? With the right physio plan, you can get your mobility back! Stretching, strengthening, and guided rehab can make a huge difference.

How we assess frozen shoulder:

- Postural assessment
- Biomechanical assessment
- Neck and thoracic assessment
- Neural tension assessment
- Strength and range of movement assessment

Possible treatments may include:

- Thoracic and neck mobilisations
- Soft tissue massage
- Shoulder mobilisations
- Accessory joint movements
- Pulsed electromagnetic field treatment
- Acupuncture
- Taping
- Thoracic manipulations
- Cryotherapy

Exercise rehabilitation is important including:

- Pulley exercises
- Pendular exercises
- Mobility exercises
- Home cryotherapy

If your shoulder has been bothering you for a while, please don’t ignore it; early treatment is key. Let’s get you moving again! Call us on 01634 817242.

14/05/2025

Achilles tendinopathy is a common overuse injury caused by repetitive or intense strain on the Achilles tendon, the band of tissue that connects your calf muscles to your heel bone. This tendon is used when you walk, run, jump or push up on your toes.

The symptoms of Achilles tendinopathy may include:

- Pain in the heel and along the length of the tendon when walking or running
- Tenderness or tightness in the morning or after rest
- Pain in the achilles tendon when touched or moved
- Swelling and warmth in the heel or along the tendon
- Difficulty standing up on one toe
- Pain and stiffness along the back of the ankle, especially during or after activity

If left untreated, it can linger or get worse over time but the good news is that physiotherapy can help reduce pain, improve flexibility and strengthen the tendon so you can get back to moving pain-free.

How we assess and treat Achilles tendinopathy:

We would initially assess why you have Achilles tendinopathy and diagnose what the cause is, including:

- Biomechanical assessment
- Hip, knee and foot assessment including mobility, strength and flexibility
- Assess gait
- Assess back and neural tension
- Assess postural dysfunction
- Assess pronation/supination of feet

Treatment may include:

- Adjust any postural dysfunction
- Release any neural tension
- Design rehabilitation programme

Possible modalities used:

- Soft tissue massage
- Acupuncture
- Mobilisations of any dysfunctional body region
- Neural tension release
- Myofascial release
- Ultrasound
- Pulsed electromagnetic field treatment
- Taping

Finally our physio will prescribe exercises to carry out at home.

Don’t ignore that ache; early treatment makes all the difference! If you are experiencing pain in this area and would like to book an assessment, please call 01634 817242.
