I recently had a hysteroscopy under sedation and it was interesting to be on the other side of the fence.

Everyone was very kind, respectful and professional. The anaesthetic worked a treat as I experienced no discomfort or any pain during the procedure. My overall care was exemplary.

I thought I’d share my experience as more often than not all we hear are negative experiences about the NHS. A huge thanks to the team at the L&D short stay surgical unit.

While I’m here;

With increasing use of HRT as doctors we are increasingly seeing women with persistent unscheduled bleeding on HRT, one of the side effects. For most, it’s a question of tweaking their HRT treatment and the bleeding settles, but for those whose symptoms persist, further investigations are necessary.

A hysteroscopy is one of the investigations done to assess the lining and inside of the uterus to rule out abnormal changes or cancer of the lining of the womb (endometrial cancer).

This procedure is painful if done without some form of anaesthetic; however, as individuals we have different pain threshold. Some of my patients have ‘managed’ just on paracetamol and ibuprofen before the procedure, but some have found it extremely painful despite the painkillers.

I opted to have sedation as I wasn’t prepared to endure unnecessary pain.

We all know about gender pain gap; yup, it is a thing, whereby as women we are expected to tolerate pain. Our experiences of pain are either dismissed or downplayed, which in turn leads to delayed diagnoses of conditions such as endometriosis, fibroids, to name but a few. There are a myriad of factors that feed into this phenomenon, not least the lack of research in women’s health or the social and cultural expectations around women’s tolerance of pain.

If you are referred to have a hysteroscopy, you should be offered different options of pain control, if not ASK!

You don’t have to grit your teeth and tolerate the pain!

Not only has elinzanetant be shown to be effective for moderate to severe hot flushes and night sweats, it may also help improve sleep!

Another option for women who prefer not to have HRT or HRT is contraindicated.

Let’s hope it is not too long before we can prescribe it on the NHS

Sundays for me are usually about unwinding and catching up with myself. If I’m not in the garden, you’ll find me in the kitchen making something!

This morning I made a batch of granola or quinola shall I say, which is just so scrumptious I could eat it all day long!

All the ingredients are usually what I have in my cupboard and I’d say it has a cup or half a cup of each of these;

Cooked multicoloured quinoa
Chopped Almonds, Walnuts and Cashew nuts
Pumpkin seeds
Sunflower seeds
Chia seeds
Flax seeds
Goji berries

All the above mixed with one cup almond milk and then roasted for about 30 minutes at 140 degrees.

Then I added half a cup of dried blueberries and cranberries and a sprinkle of cinnamon.

It’s packed with protein, fibre, good fats, calcium and many other vitamins and minerals. What’s not to love!

Give it a go and let me know what you think. You don’t have to use all the ingredients I’ve used, just experiment and you just might be surprised, in a good way! 🧡

Snippets from the conference. Here we go!

* Bone health is essential during the menopause because of significant bone loss which can lead to osteoporosis and increased risk of fragility fractures

* Fragility fractures are fractures sustained from a fall at ground level, for example if you trip and fracture your wrist or hip.

* Fractures affect mobility, quality of life and can increase risk of death.
They are also a burden on society and the health service.

* It is important to be aware of your risk factors because as most are modifiable eg smoking, va**ng, alcohol 2units or more, lack of exercise, unhealthy diet

* The more risk factors you have the higher risk of fracture you have

* A FRAX assessment is a tool that can be used to calculate the probability of fracture in 10 years.

* It should be done in postmenopausal women with a clinical risk factor premature menopause. It is a simple and well validated tool www.fraxplus.org

* Low risk women are given lifestyle advise and those with intermediate risk are referred for measurement of bone mineral density

* Those at high risk should be referred for further investigations or specialist care

* Osteoporosis is diagnosed by measuring bone mineral density using a DEXA scan

* Osteopenia indicates low bone mass which is before osteoporosis.

* Most fractures occur in those with osteopenia.

* It is important to identify women with osteopenia as this offers a window of opportunity to reduce the risk of osteoporosis and fracture.

* Prevention is always better than treatment!

* Hormone therapy is protective and should be considered for those at risk work with menopause symptoms and strongly recommended in those with a premature menopause/ primary ovarian insufficiency (POI)

* Non hormonal treatments such as Bisphosphonates are available to treat those with osteoporosis.

* Top tips to improve your bone health

✅ Healthy nutrition dense diet
✅ Regular weight bearing and muscle strengthening exercises eg walking, dancing, running, pilates, yoga
✅ Adequate dietary calcium 700mg/ day
✅ Adequate vitamin D 1000IU
✅ Stop smoking, even va**ng!
✅ Alcohol less than 2 units /day

My heart went out to as I listened to his post match interview yesterday.

This is a man telling the world that he is struggling with his mental wellbeing. This is a man describing symptoms of depression.

I was proud of him for putting it out there and being open about exactly how he was feeling. Men don’t often do that because of the stigma.

Tennis can be such a lonely sport.

I truly hope those around him are listening and will support him.
I truly hope that he WILL seek help.

If you are struggling with your mental health, please talk to someone. There are organisations out there who can support you. Don’t suffer alone in silence.

I am a highly experience GP, wellness coach and mindfulness teacher having recently completed a Masters degree in Mindfulness Based Cognitive Therapy (MBCT) at the University of Oxford. We offer mindfulness courses, workshops and coaching for health professionals. We also work with teams within organisations to offer tailored, holistic and effective programs focusing on lifestyle and wellness.
