**Do you suffer with pain at night? **

Pain at night can affect the amount of sleep you get and the quality of sleep. It can also significantly affect quality of life.

Not only do you need a good nights sleep to function well, it also helps with the healing process. This means that sleeping well is very important.

Types of night pain…

Positional night pain - this is the most common. You might wake up with pain, but if you change position, you are able to get back to sleep. Sometimes you may need to take a pain killer to aid this.

Pain in the second half of the night - this could mean there is inflammation involved. Our central nervous system has a rest in the first half of the night, but as it starts to wake up again, your symptoms can start to return. This pain will be persistent.

There are various conditions which are inflammatory in nature and it is important you see a professional regarding what could be the cause.

Unremitting night pain - this type of pain is more unusual, and could mean something more serious is going on. This pain will not only stop you sleeping, but will make you get out of bed because of its severity. There will be no position that helps. This type of pain needs to be assessed by a professional quickly, ideally your GP initially.

If you are experiencing night pain that is effecting your sleep, then get it checked out by a health care professional quickly who can help guide treatment.

** Cramp **

Cramp is a painful involuntary contraction of a muscle. Some common causes of cramp are:

- dehydration
- Weak muscles
- Fatigued muscles

Cramp can often be reduced by addressing the above issues and seeing a physiotherapist can help guide you through this process.

There are also medical conditions that can cause cramp, and if you are concerned your cramp may be linked to a medical condition, then it is important to discuss this with your GP.

**Have you ever suffered with nerve pain?**

There is a system in the body called the gut-microbiota-brain axis which is a system required to help reduce nerve pain such as sciatica.

It has been found that improving your gut health can help maintain this system. There are various ways you can improve your gut health, with exercise being one of them.

Although it is unlikely good gut health can solely resolve nerve pain, it is certainly part of a bigger picture which is beneficial to address and can aid recovery.

So many conditions in the human body, musculoskeletal and non-musculoskeletal, can be improved or resolved with exercise, whether that be directly or indirectly.

So that makes exercise win again folks!

Can you change your routine to fit a bit more exercise into your life?

The role of a ligament is to hold two bones, and therefore joints, together.

The anterior cruciate ligament (ACL) is one of two cruciate ligaments that sit on the inside of your knee to prevent your thigh bone and shin bone from moving apart.

The ACL is usually injured during a sudden change in direction. This can be with the foot fixed on the ground or during impact/landing.

You can tear this ligament or sustain a full rupture.

It was once thought that you would need surgery to a ruptured anterior cruciate ligament in order to get you back to your previous function.

There has been interesting research on ACL injury recently such as the paper below which demonstrated that ACL ruptures can spontaneously heal without surgical intervention, reaching a good functional level at 2 and 5 years.

This is a reminder that it is always worth getting a professional assessment to help guide which route may be best to take. Although the general belief is that surgery is likely required, this may not be the most appropriate approach.

This also highlights how clever the human body is! 🤯

Physical inactivity is the fourth leading cause of death globally… would you take an exercise pill to mimic the benefits of exercise without physically having to move!?

This pill does not exist yet and it is unlikely, given the multiple systems affected by exercise, that one pill could successfully do this.

BUT would you take it if it did!? 💊

This paper MRI’d the knees of 115 uninjured adults with a median age of 44 years old. 97% of knees showed abnormalities.

57% had cartilage abnormalities under the knee cap

30% had meniscal tears.

21% had tendon abnormalities

3% showed partial ligament ruptures.

Is an ‘abnormality’ an ‘abnormality’ if you are pain free and have no loss of function?

Hopefully becoming more comfortable with the knowledge that these findings are in uninjured people, may help to reduce the anxiety surrounding ‘abnormalities’ in our joints

It has been proven that people feel happier when out in nature. It is therefore not surprising that if you are inside more or this freedom is taken away from someone, it will effect their health.

In the 1970s there was some research carried out by Ernest Moore looking at the environmental characteristics at the state prison of Michigan and their influence on healthcare service demands of the prison population.

It was found that those who had a view of farmland on one side of the prison, compared to those who had no view of farmland on the other side of the prison were less likely to get mentally or physically unwell.

For those of us who have a choice, it is all too easy to slip into a habit of spending whole days without leaving the house, especially now more of us are working from home and especially this time of year.

Using the environment is a cheap and effective way to stay healthy. Have a think how you could change your routine to spend more time out in nature! 🍂

There is a misconception that MRI scans can diagnose the cause of back pain.

Less that 10% of lower back pain comes from nerve irritation or compression. Less than 1% of back pain is from a more serious source (infection, fracture, cancer or cauda equina).

MRI scans can be useful at diagnosing these things.

Approximately 90% of lower back pain (called non-specific lower back pain) is multi factorial and an MRI scan is unlikely to be able to detect which structure could be causing your symptoms.

Physiotherapists are trained to detect which category your back pain falls into and treat appropriately.

With non-specific lower back pain, the research tells us that exercise therapy, along with addressing any lifestyle factors that may be contributing will put you in the best position to manage your back pain long term.

If you have back pain and feel you could do with some help and guidance, book yourself in to see a physio who will be able to help.
