🌟 Meet the Team Tuesday: Social Prescribers! 🌟
Today we are introducing - Sarah McDonaugh, Matt Burton, and Beth Merrett – our amazing Social Prescribers that work within Selby Town PCN! 👋
Social Prescribers play a unique role, bridging the gap between clinical care and social support, helping you live your best life both inside and outside the clinic.
They can support you with:
💛 Emotional wellbeing – a safe space to talk, reflect, and explore what might help.
🤝 Social isolation – building confidence, meeting new people, and connecting with local groups or activities.
🏥 Long-term conditions – practical support for managing daily challenges and improving quality of life.
🏠 Housing & financial worries – guidance and links to the right local services.
🏃 Lifestyle changes – from getting active to discovering meaningful hobbies.
👩👧 Carer support – helping carers access resources and support.
🌐 Community connections – linking you with groups, volunteering, and local services.
💼 Employment support – advice for staying in work, returning, or exploring a new career path.
Beyond the practice, Sarah, Matt, and Beth are busy in the community, running projects, working with schools, and hosting activities like the Wednesday Wellbeing Walks at Selby Leisure Centre, 10am.
