THE SHOULDER PAIN THAT WASN’T A SHOULDER PROBLEM.

A CASE STUDY OF A PATIENT WHO PRESENTED WITH A PERSISTENT SHOULDER PAIN FOR 8 MONTHS

This patient came to see me in complaining of shoulder pain which ‘wouldn’t go away’

It all started 8 months ago during his working day as an undertaker. He was part of a team carrying a heavy coffin and he could feel the weight ‘crushing down’ on the top of his shoulder. He had to stay in this position for more than 5 minutes. It was very painful for weeks afterwards. It had improved initially but been static for months.

On examination, his shoulder had good range of movement and wasn’t weak. He actually felt better with the arm above the head. Pushing his shoulder down to mimic the carrying position worsened the symptoms. His neck was stiff in the mid section. He had a history of neck stiffness.

My clinical thoughts;
- he’d not overly stressed the shoulder on the day of the injury.
- BUT he had placed prolonged stress on the nerves as they pass from the neck, to the shoulder and down the arm.
- His shoulder was functioning fine and showed no signs of restriction or weakness on testing.
- He reported a history of neck stiffness and indeed the mid section of his neck was stiff.

My diagnosis:
Cutaneous nerve irritation (small branches of a nerve that supply sensation to the skin)

My treatment:
Hands on mobilisations to the 3rd, 4th, 5th and 6th vertebrae.

The result:
90% recovery by his second attendance. He wanted to attend a 3rd time to be sure ‘it isn’t a fluke’. By the 3rd session he’d had no pain between treatment sessions.

CASE STUDY

Are you suffering with calf tightness or pain and no amount of sports massage will help?

A recent patient at Seven Hills Physiotherapy attended complaining of severe right calf cramp when in bed at night? This would then leave him with aching and tightness for days afterwards.

He’d had many sports massages but other than providing temporary relief his problem was unchanged.

Why was this? Because it wasn’t the right treatment.

Why wasn’t it the right treatment? Because the calf muscle wasn’t the direct source of his problem.

How did I know this?

1) The calf was absolutely fine under load during the day. He could lift and carry heavy objects at work without a problem.

2) The calf was completely painless to heavy pressure (pressing and touching)

3) The calf was at its most problematic when the muscle was under least stress, lying in bed.

4) On examination there was no indication of calf tightness, no pain on stretching or when loading the muscle.

5) He had a history of back pain and stiffness.

6) On examination, the sciatic nerve was much more restricted on the right than the left.

7) The lower back was stiff and to make my life much easier the calf violently twitched every time I mobilised the 4th and 5th vertebra in the lower back.

What does this mean?

The sciatic nerve was being restricted by stiffness at the 4th and 5th vertebra in the lower back. This can cause neural ischaemia (reduced blood flow and oxygen to the nerve). When in bed his blood pressure and heart rate drops so there’s even less blood flow to the nerve.

What did we do to resolve the problem?

The idea was to resolve the stiffness in the back to allow free movement of the nerve. Hands on treatment involved progressive joint mobilisations combined with exercises to maintain lower back flexibility and increase nerve mobility.

The cramps began to reduce and at our last session he had not suffered any cramp or tightness in the calf for 6 weeks.

Left sided chest pain and difficulties breathing ‘making life a misery’

🙍🏻‍♂️ 27 year old gentleman. Works as a roofer and is an ex amateur boxer.

👉🏼 Started with very specific chest pain 2 years ago as 1a. Progressively worsened and now finding it difficult to breath.

👉🏼 Struggling at work as a roofer and exercise is difficult due to pain and breathing issues.

👉🏼 1b neck and spinal/ shoulder blade pain more recent onset only 1 month prior to attending in late July 2022.

🚑 x2 ECG’s and chest X-ray all negative.

👐🏼 Hands-on in depth assessment identified stiffness at left ribs 4-6 and the attaching thoracic spine vertebrae. Stiffness at these levels reproduced the patients chest pain.

👐🏼 Treatment consisted of ribs and thoracic spine mobilisations and high velocity manipulations. Also left shoulder girdle rehabilitation was required to stabilise the left shoulder following many years of boxing induced issues.

Result: 1 assessment and 5 treatment sessions and this gentleman is now 95% back to normal following 2 years of misery at a young age. He has been discharged with the expectation he will be fully cured in the next few weeks.

Do you have neck and/ or spinal symptoms that you’re unable to ease no matter what you try?

So did this 41 year old lady, for 15 long years.

Previous care from the chiropractor and physio provided only short term relief at best. She had resigned herself to a lifetime of pain.

I like this case, not because it was particularly challenging or difficult to reason through but because the patient arrived with such modest goals and left 100% cured after an assessment and 5 treatment sessions over 3 months.

Given the longevity of the problem and failed treatments all she wanted from me was guidance on what she could do at the gym and what she needed to avoid.

It quickly became apparent that so much more could be achieved.

Treatment included;
👉🏼 Joint mobilisations and high velocity grade 5 manipulations to the neck, thoracic spine and ribs.
👉🏼 Progressive deep soft tissue therapy
👉🏼 Exercises to improve flexibility and stabilise the shoulder girdle.

After a total of 6 visits she has returned to full activity and has been free of pain for over a month 😊

Could your shoulder pain be caused by a little known nerve called the axillary nerve?

A recent patient attended with a painful shoulder which she thought had been caused by too much reaching at her work desk. It sounded very much like a shoulder rotator cuff problem until she said she felt it mainly if she was slouched on the sofa. If she sat up straight it would go away.

Examining the neck and mobilising the 5th and 6th vertebrae we were able to bring on the exact shoulder pain.

Why? The axillary nerve (as seen below) supplies sensation and motor impulses to the muscles of the shoulder. It’s journey through the body starts at the 5th and 6th vertebrae in the neck 😉 Stiffness at these joints, combined with mechanical compression caused by the poor neck posture on the sofa caused irritation of the nerve and therefore pain.

2 sessions of neck mobilisations and exercises and the patient was pain-free and able to move the shoulder freely through range 👍🏼
