This is 3 months of regrowth in a patient with alopecia areata.

Alopecia areata is an autoimmune condition where the immune system targets the hair follicle, pushing hairs into premature shedding.

After assessment in clinic, I referred her to a dermatologist. She was prescribed a JAK inhibitor.

JAK inhibitors (Janus Kinase inhibitors) work by blocking the inflammatory signalling pathway involved in autoimmune attack. By reducing this immune activity, follicles are able to resume normal function.

These medications are:
• Prescription-only
• Specialist monitored
• Not suitable for every case
• Reserved for moderate to severe presentations

Correct diagnosis determines correct pathway.

Trichology is not about selling products.

If you’ve ever wondered what I’m looking for when I examine your scalp with a trichoscope, this will give you a clear overview.

A trichoscopy examination is a detailed, non-invasive assessment of the scalp and hair follicles. It allows me to understand what is happening beneath the surface, supporting an accurate diagnosis and a tailored treatment plan.

During trichoscopy, I look for:
• Hair shaft diameter – whether the hairs are uniform or showing signs of miniaturisation
• Follicular units – how many hairs emerge from each follicle
• Follicle miniaturisation – a key indicator of androgenetic alopecia (pattern hair loss)
• Empty follicles or yellow dots – often seen in conditions such as alopecia areata
• Redness, inflammation or scaling – to identify issues such as dermatitis, psoriasis or sensitivity
• Broken hairs or black dots – indicators of breakage, traction or autoimmune shedding
• Blood flow and capillary changes – providing insight into inflammation or vascular involvement
• Scalp barrier health – dryness, irritation, buildup or product reactions
• Regrowth indicators – vellus hairs (baby hairs), short anagen hairs and early signs of recovery

Trichoscopy allows me to see far more than the naked eye can detect, providing clarity, answers, and a clinically informed plan for your hair and scalp health.
