You may notice a test result appear in your NHS app referring to patient need groups or risk stratification. This is a simple way in which we categorise patients based on their specific health needs. It helps us as a practice to understand the individual needs of our patients based on their health and wellbeing to support personalised care and ensures you get the right support in a timely way. No action is needed from you.

Memorial Medical Centre is a five Partner Practice (four GPs and a Nurse Practitioner) based in Sittingbourne Kent. Our aim is to provide care to the whole person whatever the nature of their medical problem.
Opened in 1995, the Practice strives to offer all its patients up-to-date care without losing sight of the traditional perspective. We serve more than 16,000 patients with quality care.
We are also a Training Practice helping young medical professionals launch their careers.
The purpose built surgery is located within the grounds of Sittingbourne Memorial Hospital, adjacent to the Memorial Pharmacy, providing our patients with excellent access to a full range of community services.
The premises are suitable for disabled and/or wheelchair patients and has baby changing facilities.
All patients with the Practice are allocated a named GP and you can check who your named GP is anytime you contact the surgery. You are encouraged to see this GP for every consultation unless they are not available for appointment bookings at that time.
New patients are informed of their named GP when attending for a new patient health check.
