It is important everyone over the age of 14 who is on their doctor's learning disability register has an annual health check.

They can help you stay well by talking to a doctor or nurse about your health and finding any problems early.

If your doctor's practice has not already offered you an annual health check, you can ask them if you can have one.

️"Do you include me?" - Rahima's story

💬 “My name is Rahima Begum. I use this eye gaze computer to communicate with you. I need support from carers and family to be able to express myself better.”

In 2020, Rahima went to hospital for a chest infection. She was left out of conversations about her care, and decisions were made without asking her what she wanted.

“If the Government can make sure the NHS works for people with a learning disability then we can get the help we need, and there will be less mistakes.”

It's time to make sure every voice is heard.
Our Easy Read form is designed to make it simpler for you to tell the NHS how things can be improved for people with a learning disability.

Have your say today here, https://brnw.ch/21wQDIR

Image description: A photo of Rahima wearing a green jumper, looking towards the camera. There is a purple border around the image and white text that reads "Disabled people are unique and complex. We want healthcare that is tailored to us.". There is also text that reads underneath "What changes do you want to see in the NHS?"
