Your Health Notts and Active for today are services used within Nottinghamshire to provide help and support to patients on a range of lifestyle interventions such as weight management, exercise and smoking cessation. Both services will be present in the waiting room at the surgery between 9am and 12pm tomorrow.
If you would like to speak with them or self refer, you can do so by attending between these hours.
They will be able to undertake height, weight and blood pressure measurements on site also.
