We will create a person centered plan with the individual and any other professionals involved in their care to ensure their needs are being met so they can continue to live as independently as possible within their own home.

This can be from meal preparation, assistance with personal care, medication administration and companionship. We will also work alongside other health professionals in the areas such as GP's, District Nurses, local authorities and charities to ensure the individual is receiving any extra support available to them.

We promote independence and empowerment with each individual to ensure they are fulfilling their goals.
