Our community neuro rehab team (CNRT) are proud to make a difference in our community every day. They provide specialist, multidisciplinary rehabilitation support for adults with neurological conditions such as stroke, multiple sclerosis, Parkinson’s disease, motor neurone disease, and brain injury. Their goal is to empower individuals and their carers to self-manage their conditions and improve quality of life.

This year marks the 80th anniversary of the Royal College of Speech and Language Therapists, and we’re proud to celebrate alongside our incredible team of therapists and those around the country, who make a real difference every day.

Our speech and language therapy teams support people of all ages with communication and swallowing needs.

For children, they help with speech sounds, understanding and using language and feeding skills. They work closely with families, schools and nurseries to give every child the best start in life.

For adults, they provide therapy for communication difficulties after stroke or neurological conditions, and support safe eating and drinking for those with swallowing problems. Care is delivered in homes, clinics, hospitals and care settings, tailored to each person’s needs.

Congratulations to YH Proactive Anticipatory Care locality managers Giles and Rob who, along with colleagues from SW London, have received a Collaborative Working Award for Kingston’s PAC service: a blueprint for neighbourhood health care as described in the NHS 10 year plan.

The award recognises the exceptional contribution to personalised care across London made by PAC.

YH is a key partner in the PAC model, working with colleagues across health and social care in Kingston to support people with long-term conditions, by creating tailored care plans that take into account socio-economic and environmental factors, ensuring early intervention and holistic care.
