MEET THE TEAM - Jayde

Jayde is the latest addition to the TMH Physiotherapy family and is also our first international team member.

Originally from sunny South Africa, Jayde qualified as a Physiotherapist in 2021 where she began working in local rehabilitation hospital's focusing on the delivery of high quality treatment for patients with neurological, spinal and general medical conditions.

Since then she has amassed a wealth of valuable experience for another private home visit provider in Johannesburg before emigrating to the UK in 2024 after which she was quick to join our team in 2025.

Jayde is highly specialised in the treatment of neurological conditions and is truly passionate about helping her clients to regain as much independence as possible to live a better quality of life.

PATIENT STORT - Anne

Let me introduce you to Anne who is one of the sweetest ladies i've ever met. After recurrent falls at home she unfortunately suffered a fractured femur.

Following surgery, Anne was discharged from hospital to a residential home where her only way of moving was to use a rotunda with the staff here. Due to the falls she had developed a temporary neurological condition known as psychomotor disadaptation syndrome (PDS).

Despite their best efforts, NHS therapy couldnt progress her. It was at this point her Son reached out to us for help. Well, after weekly Physio sessions with Tom she has made such incredible progress!!

Anne is now independently mobile >100m with a four wheeled frame and she is learning to walk unaided again. She can do stairs, car transfers and has even had an outing with Tom to a local cafe, which was her first time out in over a year! (SEE COMMENTS FOR PICTURE)

Our next goal is to use a walking stick independently and to have enough endurance ti walk around a local park.

Its been a few months since our last post but we hope everyone had a fantastic Christmas and New Year.

PATIENT UPDATE - Margaret

Well its been a couple of months since we last posted about Margaret but what a difference!!

This lovely and determined lady was originally admitted to hospital following a fall; from which she sustained a fractured left hip as well as a dislocated left shoulder.

On our first post we had managed to progress her from using a hoist to a rotunda with some early signs of stepping. With further weekly Physio sessions Margaret can now not only walk with her frame around the house, but she can also access outdoors by completing the steps to her front door.

This is what can happen when a specialist Physio service and a top level care team from Home Instead Ashby, Burton & Derbyshire work in partnership to truly provide a bespoke rehab service for our elderly clients.

Just in this month!!

Latest research from 2025 shows that high quality home based specialist physiotherapy, carried out face to face, is significantly more effective at improving mobility and functional independence in patients following stroke.

This is when compared to hospital based stroke rehabilitation or the use of unsupervised home exercise sheets provided by physiotherapists.

The comparitive improvements are at least 30% across all outcome measures with some scores showing up to 5x the improvement.

Its great to see that high level research is backing up what we do here at TMH Physiotherapy.

It was such a great pleasure to be able to provide Alexie with her home physiotherapy following such a traumatic ordeal which that has been publicised by the BBC recently.

Facing months in a wheelchair, Alexie contacted us from hospital desperately searching for specialists to provide the much needed rehabilitation needed so that she could return home rather than going to another hospital unit; the idea of which was making her so upset and feeling hopeless.

After an assessment and rehab planning with Tom, Alexie then received weekly sessions with Jess and the results speak for themselves.

Alexie has regained her ability to walk! She is also back to running and football training in such a short time.

It's been a pleasure to support her and just shows what can happen with you combine a bespoke specialist neuro rehab team with a young determined client.

MEET THE TEAM - Jack Koczalski

Jack first started his career as a Sports Therapist after gaining a BSc Sports Therapy degree in 2018 from Leeds Beckett University.

In this role he worked at various football clubs including Nottingham County FC and Nottingham Forest FC where he provided rehabilitation to many elite level athletes.

After this, Jack joined the NHS to work as a Rehabilitation & Exercise Practitioner in the musculoskeletal department before undertaking a MSc Physiotherapy masters degree with the University of Lincoln in 2020.

Upon graduating as a registered Physiotherapist in 2023, Jack continued to work within the NHS; gaining valued experience within inpatient stroke rehabilitation.

Jack has a special interest in Neurological and Stroke rehabilitation and has a passion for helping people to achieve their physical goals in a time of need.

He joined TMH Physiotherapy in 2025 and has already proven his ability to follow in this passion by successfully rehabilitating many of our clients.

PATIENT STORY - Beryl

Let me introduce you to Beryl. This lovely lady at the grand age of 98 was referred to us initially for a home assessment in order to see if she can use a rotunda to stand after being nursed in bed for months following various hospital admissions and failed home therapy.

Beryl has advanced Alzheimers dementia as well as a posterior stroke and as such deals with many challenges for both her and her loving family that support around the clock alongside the amazing carers from Home Instead Ashby, Burton & Derbyshire .

Because of her dementia, nobody initially thought that she had any potential to regain mobility. But we were convinced that the part of her brain which deals with motor planning and postural processing was not yet affected...and after all, we if anyone, will always give a client the best chance possible especially if they have been left behind before.

So after some postural processing correction and neuro-rehabilitation this is Beryl after only a few sessions. Today not only can she walk a few steps but she has walked from her bed throughout her house to her much loved conservatory to have a cuppa in the sun with Tom.

Her mood, cognition and general health has drastically improved and whilst she still has some obvious issues to battle, her quality of life is already drastically better and we are here for the journey side by side with her.
