Diamond Heart Health Care is
looking for a committed and highly organised Coordinator to join our Wakefield team. This role is ideal for someone who is passionate about delivering high-quality care, supporting staff performance, and ensuring smooth day-to-day operations.

✨ Key Responsibilities

Coordinate staff rotas and ensure appropriate shift coverage.

Support care workers in delivering safe and effective care.

Develop and maintain individual care plans.

Conduct staff supervisions, appraisals, and inductions.

Work closely with the Registered Manager to maintain compliance with standards and legislation.

Liaise with families, professionals, and external partners to ensure clients receive the support they need.

To collaborate with the admin team (documentation, reporting).

✔️ Requirements

Experience in health and social care coordination (preferred).

Strong organisational and communication skills.

Ability to work under pressure and manage multiple priorities.

Knowledge of CQC standards is an advantage.

A positive attitude and passion for supporting both clients and staff.

🚗 We’re Hiring! Domiciliary Care Team Coordinator – Wakefield

Diamond Heart Health Care is growing in Wakefield and we need a caring, motivated Team Coordinator to support our clients and staff.

⭐ The Role

Support and guide care staff so they work safely and effectively

Help manage the service in line with standards and legislation

Be a positive role model and leader

Develop person-centred care plans

Lead inductions, team meetings, and supervisions

Work with professionals to make sure clients get the right support

You’ll also provide hands-on care when needed — helping clients live with dignity in their own homes.

🚨 Essential: Full UK driving licence + own car (to travel between clients).

✅ Other Requirements: Kind, reliable, good communication, flexible, right to work in the UK.

💷 We Offer: Competitive pay, mileage allowance, flexible hours, training & development, and a supportive team.
