Part 1 -

Bob, 89 years old & from Somerset, broke his leg in February when he fell in his garage. Here is his story…

After a stay in hospital, he returned home but required a lot of help to carry out day-to-day tasks like washing, cooking, getting into bed and simply moving around the home.

This is where Bluebird Care came in. Bob initially received 4 visits a day from our Discharge to Assess (D2A) Enablers - they cooked him meals, helped him get dressed and the ready for bed. They got Bob a trolley to help him walk and guided him through his physio exercises.

As Bob got better, the level of support eased off until he was back to independent living.

As a Discharge to Assess (D2A) Bluebird Enabler, every day can be different.

In one call you’re completing an assessment alongside NHS keyworkers for someone who has just left hospital in need of a high level of care.

The next, you’re checking in with a customer who can now dress themselves and is almost fully independent.

Each call is different but all are highly rewarding and goal driven.
