“Julie instantly recognised I was dysregulated and not in control of my nervous system. Anxiety and nervous system dysregulation are VERY common in people with vestibular problems — and she has taught me that I can use physical triggers (breathing primarily) to control my nervous system, something that was previously very alien to me.”

This patient came to us following vestibular neuritis. She had already done VRT with an audiologist — it had helped, but only so far. Her ENT specialist referred her to Hito to continue.

What she describes in this review is the piece of vestibular treatment that is almost never addressed anywhere else. If your dizziness is consistently worse under stress — and better when the pressure lifts — your nervous system is a significant part of what’s driving it. 🌀

This is called central sensitisation. It’s a neurological state in which the brain becomes hypersensitive to incoming signals — including every balance signal your vestibular system produces. In that state, environments that were previously unremarkable become genuinely overwhelming. The tube. Crowded spaces. Fluorescent lights. Screens. Not because they are dangerous, but because the nervous system has been calibrated to treat them as threats.

Standard vestibular rehabilitation doesn’t address this. It retrains the balance system. It doesn’t regulate the nervous system that amplifies every signal the balance system produces. And without addressing both simultaneously, the gains from physical treatment plateau.

You were supposed to feel better by now.

You took it easy on Saturday. You didn’t push too hard. You stayed away from the things that usually trigger it. You gave your body the rest it kept asking for.

And on Sunday evening — or Monday morning — the dizziness is back. Sometimes worse than it was on Friday.

Here’s what’s happening. Your vestibular system does not operate in isolation from your stress system. They share the same wiring. One of our patients came to us with jaw pain at 8–9/10. Neck pain at the same level. Tinnitus that had become a constant background presence. Six weeks into the Hito Programme — here’s what the data showed. 😵‍💫

The jaw pain. The neck tension. The headaches that arrive every Monday. They’re not three separate problems — they’re one cycle. And understanding it changes everything about how to break it.

Stress triggers jaw clenching — often unconsciously, often all day long, not just at night. That clenching sustains muscular tension in the masseter and temporalis muscles, which refers to pain into the temple and skull. The jaw tension pulls on the upper cervical spine, which shares the same nerve pathways and feeds directly into the headache pattern. The headache increases the stress response. The stress drives more clenching.

Most treatments address one piece of this. A night guard at night. Physiotherapy for the neck. Pain relief for the headaches.

The cycle continues because it needs all three dimensions addressed together.

One of our patients came to us in the middle of her second episode of labyrinthitis. Daily dizziness at severity 5–6. Constant tinnitus. Three to four hours of sleep a night. Unable to exercise. Withdrawn from friends and social life entirely. 🧠

As a working professional, the impact wasn’t just physical. Her confidence, her identity, and her capacity to do the work she loved were all quietly unravelling.

Most vestibular treatment focuses on the inner ear. Repositioning manoeuvres for BPPV. Balance exercises. Sometimes medication. These all have their place. But they address one dimension of a condition that almost always involves three.

Your brain uses three signals to know where you are in space — the inner ear, the neck, and the eyes. When these stop communicating clearly, dizziness follows. When your nervous system is also running on high alert from chronic stress, it amplifies every signal it receives. To everyone around you, you looked completely fine. What they didn’t see was the amount of effort it took. The quiet concentration just to walk in a straight line. The decision to stand near the wall, just in case. The relief when you finally sat down.

Living with dizziness is exhausting in a way that’s almost impossible to explain — because from the outside, nothing looks wrong.
