We complete skilled assessments and reviews and provide you with right sized and timely support planning recommendations. We rigorously apply eligibility criteria, building on the persons strengths and support networks and we promote the use of universal services. Where public funding is required to meet needs, we ensure that best value principles are applied to support plans, whether the person resides in the community or in care homes. We also support people to set up Lasting Powers of Attorney - including application, witness and signatory and registration processes to reduce the need for Best Interest meetings. PSCS delivers the service to local Authorities and Health authorities covering the whole South West peninsula of England. We will be tendering for larger contracts as we grow and attract a wider supply of committed and skilled Social Work practitioners sub-contracting a range of professional assessments to deliver the service. Social Care Core Assessments - Identifying needs and risks, recommendations for Care and Support Plans and Care Reviews for vulnerable adults. Right sizing care and support packages - Based on rigorous application of national eligibility criteria, building on the persons own strengths, the informal support of their own social capital / community assets and effectively signposting to or brokering universal sources of support. Where people have identified needs requiring public funding to meet them, we scrutinise care and support plans to ensure needs are met and strengths are maximised. We ensure services are provided in a cost effective manner and that specific time allocated to care delivery and service user supervision is balanced with their rights to maximise opportunities for privacy and independence. Carers Assessments - Either separately or as part of the service users assessment. Mental Capacity Assessments - Applying the functional test of capacity as defined in the Mental Capacity Act 2005. Continuing Health Care - Screening assessments for NHS Funded. (CHC) - Providing independent Social Work input to CHC appeals and reviewing nursing care packages of care with a view to resizing healthcare plans provided under Continuing Health Care. (Jointly with health professionals). Lasting Power of Attorney applications – A separate subsidiary service to assist the Creation, Registration, Witness and Certification of Lasting Power of Attorney documents. Additional Specialist Assessments - There is scope for product development to expand, for example to offer Occupational Therapy, Best Interest Assessments as part of Deprivation of Liberty (DOLS) safeguards and independent assessments of potential foster parents. (Form F)
Particular requests for specialist assessments may be sourced as required. The service is also accessible to those who may be ‘ineligible’ to receive statutory funding / services and those 'self funders' who pay directly for their own care services.
