Combined / Single session EUS guided FNB for tissue acquisition & ERCP for biliary decompression via stent insertion, in a patient with distal biliary obstruction secondary to a pancreatic head mass.

EUS (HPB)
FNB x 3 performed at the 4cm pancreatic head mass lesion, using a 22G Acquire-S FNB needle (via stomach and D1 approach)

Followed by,

ERCp:
Wire guided CBD cannulation -> Cholangiogram = Distal CBD stricture -> Sphincterotomy -> Biliary brushings -> FcSEMS deployment

ERCp for duct clearance in a patient admitted with obstructive jaundice, secondary to a 3cm choledocholithiasis.

Ampulla on edge of a diverticulum -> Wire guided CBD cannulation -> Cholangiogram = A large oval filling defect in dilated CBD -> Decent size Sphincterotomy -> Balloon trawl using 18mm extraction biliary balloon and a massive CBD stone retrieved. Duct clear on occlusion cholangiogram.

Interesting ERCp case with more than 35 stones retrieved from patient's CBD + CHD + RPHD + RAHD + LHD

Wire guided selective biliary cannulation -> Cholangiogram = Multiple filling defects in Common Bile Duct + Common Hepatic Duct + Right Anterior & Posterior Hepatic Ducts + Left Hepatic Duct -> Biliary sphincterotomy -> Sphincteroplasty -> Spyglass Cholangioscopy guided EHL of the distal most wedged complex stone -> Selective wire cannulation in each hepatic duct -> Biliary balloon trawls using extraction biliary balloon and all stones retrieved -> Clear duct on occlusion cholangiogram.

Previously failed ERCP at another centre twice, due to complex choledocholithiasis.

Leinzett cholangioscope assisted Electro-hydraulic Lithotripsy (EHL) performed on the large complex CBD stone, followed by balloon trawls with extraction biliary balloon to retrieve broken pieces. Occlusion cholangiogram confirmed duct clearance.

One with a pancreatic body/tail cyst and the other with a pancreatic head cyst.
Fluid sample sent for Cytology, Glucose, Amylase and CEA.....

ERCp performed for duct clearance, in a patient admitted with biliary sepsis secondary to a blocked plastic stent inserted initially at another centre due to multiple large choledocholithiasis, which couldn't be cleared on the index procedure.

Previously inserted DPS removed with FB removal forceps -> Selective biliary cannulation -> Cholangiogram- Multiple filling defects in CBD -> Sphincteroplasty with 10-12mm CRE biliary balloon -> Ten CBD stones retrieved via extraction balloon trawls -> Occlusion cholangiogram = Clear duct
