PRIMARY CARE NETWORKS
Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often. To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs). PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve. Each of the 1,250 PCNs across England are based on GP registered patient lists, typically serving natural communities of between 30,000 to 50,000 people (with some flexibility). They are small enough to provide the personal care valued by both people and GPs, but large enough to have impact and economies of scale through better collaboration between GP practices and others in the local health and social care system. PCNs are led by clinical directors who may be a GP, general practice nurse, clinical pharmacist or other clinical profession working in general practice. Over 99% of general practices are part of a PCN, who sign up to the Network Contact DES which details their core requirements and entitlements.

If you have flu, there are some things you can do to help get better more quickly.

Flu will often get better on its own, but it can make some people seriously ill. It's important to get the flu vaccine if you're eligible.

Flu cases are higher than they have ever been at this time of year.
An average of 2,660 patients per day were in a hospital bed with flu last week. That’s enough flu patients each day to fill more than 3 whole hospital trusts.

If you haven’t already, and you’re eligible, get your flu vaccine.
The flu jab reduces your risk of becoming seriously ill this Christmas, and helps protect those around you.

Book via your GP, pharmacy or the NHS App.

Pharmacists and GPs across Staffordshire and Stoke-on-Trent are asking people to help the local NHS by only ordering the medication they need from their repeat prescriptions.

A report by the Department of Health estimates that unused medicines cost the NHS around £300 million every year and a large amount of medicine items are wasted unnecessarily in Staffordshire and Stoke-on-Trent each year. We need your help to reduce this.

It is important to check what medicines you have at home before placing your repeat prescriptions order. A significant amount of repeat prescription medicines are not used and returned to pharmacies for destruction or disposed of in other ways which are bad for the environment.

Ordering unnecessary medication can increase the risk of medicines shortages in pharmacies and we want to make sure essential medications are available to those who need them most. We want people to have the right medicine and the right care, while reducing the amount of medicine waste.

💊 Only order the medication if you need it – repeat items still remain on your prescription, even if you don’t order them every month.

💊Try keeping all medication in one safe place at home, so you can see what you have left and what you need to reorder.

💊 For each prescription, take medication from one pill packet at a time, so that you can keep a track of what you’ve taken and what you have left.

If you have questions about your prescription or need some support, speak to your pharmacy team. Unused or out of date medicines and inhalers should always be returned to your pharmacy for safe disposal.

With temperatures plummeting across the UK this week, we're sharing 8 top tips to help protect your heart in the cold. See below 👇
1.Keep Indoor Temperatures Comfortable: If possible, keep the room where you spend most of your time heated to at least 18°C. A hot water bottle or an electric blanket can help you to keep warm at night.
2. Stay Warm: Wear lots of thin layers, rather than one thick layer. This will trap the heat in and keep you warmer. A hat, scarf and gloves will also help maintain your core heat.
3. Stay Active: Keep moving while you're indoors, to build up your core temperature and boost your immune system.
4. Maintain a Healthy Diet: Have warming meals and healthy hot drinks to give your body the energy it needs to keep you warm.
5. Wrap Up: If you get angina (chest pain), wear a scarf wrapped loosely around your mouth and nose or wear a face mask, so you breathe in warmer air. This may help to improve your symptoms if they get worse in the winter.
6. Stay Informed: Learn how to make your home more energy efficient, and receive any financial help that you’re entitled to, such as the Cold Weather Payment. Citizens Advice also has lots of useful guidance.
7. Flu Vaccination: People with a heart or circulatory condition are more at risk of becoming seriously ill from flu and will be eligible for a free flu vaccination.
8. Speak to a Nurse: You can speak to a BHF nurse on the our Heart Helpline by phone, email or chatbot. Our website also has lots of information and advice on how to stay warm this winter, including money-saving tips and nourishing recipes.
