When the pelvis rotates in the sagittal plane, the position of the acetabulum (hip socket) changes as well. Thus, when various restrictions, imbalances, and compensation patterns are holding part of the pelvis in an anterior or posterior tilt, the functional length of that leg will change too. An anterior tilt, for instance, will make that side leg functionally longer, pitching your weight to the opposite side. When you think you have one leg that's longer than the other, or someone tells you so... this is usually the sort of thing that's happening.

The intervertebral discs of your spine lack a defined blood supply. They get their nutrition through what's called inbibition -- this is the movement of nutrients and fluids into your cells and body parts in the absence of a defined blood supply. Inbibition requires adequate movement. Disc herniation, degenerative disc disease, and nerve pain are all common symptoms of inadequate inbibition. Movement is life.

One of the more common causes of knee pain with running is weakness of the gluteus medius muscle, the main muscle on the lateral aspect of the hip. To check yourself, stand on one leg. The hip on the up leg side should be a little higher than the other. If not (something called the Trendelenburg sign), you may have gluteus medius weakness on the stance side.

Trigger Points - Tensor Fasciae Latae (TFL) Muscle 🚶‍♂️

The Tensor Fasciae Latae (TFL) muscle, crucial for knee and pelvic stability, often harbors trigger points leading to hip and knee dysfunction. Pain is typically felt at the greater trochanter of the hip and can worsen with activities such as walking or running. It's also common for pain to radiate down the lateral thigh to the knee, sometimes solely affecting the knee area.

Characteristics of TFL Trigger Point Pain:

- Triggers pain at the hip's greater trochanter, intensifies with activity.
- Can cause referred pain down the lateral thigh to the knee.

Anatomy of the TFL:

- Lies anterior to the gluteus maximus on the hip's lateral side.
- Originates from the iliac crest and ASIS; inserts into the IT tract.
- Flexes, abducts, medially rotates the hip; stabilizes the knee.

Functionality:

- Key in walking by stabilizing the hip and knee.
- Redirects forces, aiding lower limb alignment.

Common Indications:

- Hip pain and stiffness.
- Lateral thigh and knee discomfort.
- Reduced hip mobility affecting gait.

Causes:

- Prolonged sitting leading to imbalances.
- Repetitive motion causing overuse.
- Physical activity strain or trauma.

Advice for Patients:

- Regular stretching and mobility for the hip.
- Ergonomic adjustments for prolonged sitting.
- Heat therapy to relieve muscle tightness.
