The body is designed to heal from injuries and adapt to new loading, so why is it that so many people get pain that will not settle down? This is a conversation, just out yesterday, between Dr Rangan Chatterjee (of the BBC's "Doctor in the House" fame) and Dr Howard Schubiner on the latest from the frontiers of the pain research discussing chronic pain. For more information have a look at sirpa.org, or book an appointment to see if this approach might help you. SIRPA Howard Schubiner

Would you use ice on an injury? For decades we've thought that this was a good idea, but in recent years there's been increasing evidence that this is not helpful, with studies showing that ice can potentially hinder and delay the healing process, reduce our sense of the joint position and give a false sense of security which could make us more vulnerable to further injuring the area. So think twice before using ice - remember that the body has a fantastically clever system of healing strains, sprains and bumps and trust it to get on with it! And beware of those out-of-date web pages which tell you otherwise.

Which one of these back facts surprises you most?
Challenging the received wisdom on backs is important - what we've grown up thinking about back pain isn't always accurate - but can become a self-fulfilling prophecy because beliefs are so powerful. We base our advice and treatment on the latest research and neuroscience.

Do you know someone who's waiting to have a knee replacement? A new Danish study has shown that just 1 simple home exercise, done twice a week for 12 weeks led to two-thirds of the patients deciding that they didn't want to go ahead with surgery as they were feeling so much better! Could getting stronger muscles around your knees help you?

Will running damage my knees?

There’s lots of evidence that exercise is really good for our physical and mental health, but what about recreational running – does that cause damage to your knees? It seems not!

A recent article in the highly reputable British Journal of Sports Medicine from a group of Australian researchers reports that recreational running may actually protect us against the development of knee and hip osteo-arthritis. Interestingly, higher rates of arthritis are reported in non-runners, but also in elite runners who have elite-level training loads (although this may be related to previous injuries).

I already have some arthritis in my knee and I’m over 50 – is it ok to carry on running?

Current research says yes - but allow time between runs for the cartilage to recover. So maybe run every other day – spread the loading out over the week and not all at the weekend. I also recommend to the runners that I see in clinic to introduce change to running gradually – no sudden big increases in distance or pace. This gives the soft tissues time to adapt to the new forces – it’s kinder to the system.

Be careful too when you return to running after a break or holiday, your fitness may have been affected, so return at a lower level and build up again.

Research is on-going in this area, so we'll keep watching this space!

This video is a great summary of how the automatic part of our nervous system, called the Autonomic Nervous System works all the time to keep us safe. If this system stops functioning correctly due to stress, either in the past or in the present, we can develop pain and other symptoms as part of the response. It's just under 9 minutes long and well worth a watch!

Have you noticed that when you're learning a new physical activity such as a sport or playing a musical instrument, that you perform better after a sleep? Same applies to learning for school work! Have a good, and restful weekend!

How did you sleep last night?

Brain science has shown us that rest is where the real benefit of practice happens. The brain has time to sort and file new information and changes that have occurred. Sleep in particular has been shown to be a critical element in learning, so for better results, learn to switch off.

Wishing you all a great night's sleep...

I'm really looking forward to the release of the "This Might Hurt" Film Premiere on Saturday, 6th March at 5pm. If you have persistent pain or know someone who does, this is a documentary that follows American doctor, Howard Schubiner as he works with several chronic pain patients using a new, science-based treatment for calming the nervous system and unlearning pain. You can see a compelling trailer at the link below. There is a Q&A afterwards with Dr Schubiner, the film directors and physio Georgie Oldfield.
