My Friday mornings are all about providing supervision to HI CBT trainees and therapists.

This morning a supervisee wanted advice about planned therapy endings. This is such an important part of the therapy process. When done well, it strengthens recovery and reduces relapse rate & severity.

We talked about:
Old me Vs new me formulations
Relapse management
Repeating questionnaires
Reflective questions
Therapy blueprint
Saying goodbye
Emotions - both of the client and therapist. Yes! Even therapists can feel sad, proud, and lots of other emotions, when therapy comes to an end.

This morning I enjoyed a very wet and windy dog walk. A nice way to start the day with fresh air and exercise as a way to care for my body and mind.

In CBT I often talk about using a balance of activities in 4 main areas to help keep your mood buoyant. The categories are:

Body balance
Sense of achievement
Connecting with others
Enjoyment

I would categorise my dog walk under all of the above as I had a lovely natter with a friend, along with the exercise.

Connecting with others releases the hormone oxytocin. Exercise releases endorphins and increases dopamine & serotonin: neurotransmitters that enhance mood, reward, satisfaction and motivation.

I've found a new resting place for my headphones! 😆

I've been thinking a lot about my own space recently. Clearing space, getting rid of things I no longer need. There's nothing like a good clear out.

It's funny how clients (or in this case, clients of CBT therapists I'm supervising) filtrate into our everyday thoughts/lives.

The client I'm referring to has difficulties with hoarding which is a form of OCD. Most of us will never keep so much stuff that it will get to extreme levels of distress or danger. Nevertheless, we can still find it difficult to let go of things we hold dear, might be useful, cost a lot of money etc.

How do you decide what to keep and what to let go of?

I tend to hold onto useful things. Although, this phrenology head does not fall into that category! I've had it "donkey's years" (why do we say that? 🤷🏽). I absolutely adore it and I feel even more justified to hold onto it because it now serves some usefulness 😆

Happy Lamp

So, I finally took the plunge! Today I received the parcel I've been waiting for. It's been what seems like a loonnngggg month. The Christmas cheer is far gone, and Spring seems like a lifetime away. Despite all the vitamin D supplements and lunchtime walks, I felt like I needed more of a boost. I bought a "happy lamp".

I ordered it a couple of days ago. There was the option to choose next day delivery but I opted for the greener option as it was not an urgency (despite the January blues).

I was excited to open the box.

Super simple assembly consisting of screw together sections in a matter of minutes.

I opted for the floor lamp as my desk is already chocker-block with laptop, screen, stationery, not to mention, paperwork. You wouldn't believe I like a clear desk 😬 There is the option to place on your desk/ table by simply not using all of the poles- if that suits your needs.

Although bought primarily for it's 20k lux light therapy mode, I am impressed with the nightlight function, and additionally the colour, brightness & even timer options. Additionally, for those energy warriors amongst us, the A level energy rating is a nice bonus.

Originally it was bought it for my office. However, I am loving it's super sleek design, colour options and lighting options that I will order another for the living room as the night light provides a much softer, warmer glow than one I have had for a good few years (that I no longer think of as my fave).

It has a remote control and a nifty little remote holder so you always know where it is (as long as you place the remote back in it's holder). Can you believe batteries for the remote are also included. Well done Amazon!

Only downsides I can see is the cable. It could have been a bit more generous on the length. So unless you want to use an extension lead, there needs to be a socket close by.

I will report back with the effectiveness of the light therapy in due course, but for now, I say a £30ish well spent.

I've completed treatment with a 40 year old mum of 2 who identified with Generalised Anxiety Disorder. GAD is a common mental health condition where you often feel very anxious/worry about lots of different things.

Common symptoms of GAD include fatigue, irritability, sleep disturbances, difficulty concentrating.

These are her scores on self-report questionnaires, showing a vast improvement in symptoms over the course of 12 CBT sessions.

Additionally, and more importantly she has met all her goals. She is worrying less, letting go of needing to try to control things, and embracing uncertainty. She's so pleased with her progress. I'm so proud of her.

Hello January!

Happy New Year!

Prioritising self care is so important for our mental health and at this time of year it is often high on the list of new year's resolutions in the form of "new year, new me" related to body/fitness/life goals.

Looking after your mind, as well as your body is also important, but often overlooked.

Mental health self care can simply take the form of being more gentle and kind to yourself, learning to say "not today", allowing yourself to rest when needed, finding gratitude, accepting praise, asking for help or slowing down.

I am easing into January slowly. Anyone else?

Today is the last day I provide supervision, guidance and support for this year's university cohort. Trainee Cognitive Behaviour Therapists for children and young people will soon become qualified.

It is so sad to say goodbye to such a fantastic, motivated, down to earth, and genuinely caring bunch. I can honestly say that I thoroughly enjoyed being part of their journey as they developed new skills, learnt techniques, makde mistakes, learnt from them, learnt from each other, and grew in confidence.

It is such an intense, demanding and stressful postgraduate course. Although right now all trainees are feeling the strain of crossing the finish line, I know soon they will realise it was all worth it. It was one of the best things I ever done 15 years ago! I am so proud of each and every one of my supervisees and I know they will make fabulous CBT therapists. Congratulations Class of 2025!

Today a young person who I've been treating for OCD and helping with managing their overwhelming emotions asked me for a pause in treatment.

They have been getting on really well, making clear improvements, navigating difficult situations, communicating their needs, dealing with their OCD bully.

However, there is a demand that this time of year brings. We all understand it, don't we? Whether we are a parent, child, young person or elder, we can relate to having a gazzilion things on our to do list and numerous social events. There are lots of expectations for this time of year. Some of them we want to meet, some of them we're not so bothered about.

The fact that this young person was able to communicate to me their needs, to say, "right now I need..." is such a big deal, a big step, progress! I simply have to celebrate this.

Anyone who has had treatment with me knows I am not a 'pushy' therapist. I will not take advantage of, or coerce anyone. I will not make you feel guilty for not attending an appointment, or try to lock you into a certain number of sessions. It's all on your terms.

I can inform you of what research shows is best for outcomes and NICE guidelines, but I know that you are more than a number in a random clinical control trial. You are a person and as my client, you have a choice, a voice, control, even if you are only 14 years old!

This is something I was unable to offer when I was working within adult IAPT/ talking therapies or CAMHS. In fact, having flexibility is one of the reasons I pride myself on and this is something I NEVER ever want to change.
