Ive used an adapted model of the Starr protocol on this tear and I must say, I’m impressed with the results.

I am 1 week post long head of bicep tear and I’m back to 90-95% strength in most movements.

Cable curls 100% strength and barely a twinge

Isolated curls - outer range very tentative but getting good connection to the muscle

Chin ups really expose the facial element of this tear from forearm to lat - I can put more power through it here

I’ve also been working on some shoulder stability to compensate for loss of AP control. Mainly infraspinatus and serratus anterior.

I will definitely be using this as a first resort with clients in future.

I have hit a wrinkle. TCS London Marathon in their wisdom request that you go to London to pick up your race number.

I am away and then have a very important persons 1st birthday.

I am pleading to the Facebook gods that a wonderful someone, might be able to help me out and can pick my number up for me.

Apparently I need to sign over the deeds to my house or something, but there is a way.

This is a very well known image taken from a paper by Kjaer et al, 2007.

This is one of many papers looking into muscle atrophy in the spinal muscles following back injury. In this study the focus is on multifidus, but atrophy can be seen across multiple spinal muscles.

The great thing about this image is that it shows exactly the cascade of biological changes that occur for people in chronic pain when the injury is not managed effectively.

Injury occurs. Pain creates dysfunctional movement patterns and muscular inhibition. The injury heals. The dysfunctional movement and muscular inhibition persist. This results in chronic pain states and high risk of injury reoccurrence.

Initial injury occurs when a biological tissue is pushed to breaking point. This can be done chronically with micro loading on a daily basis beyond the point that the body can accommodate and heal from. Or, it can happen acutely with a severe injury or impact.

The key concept at that stage is allowing the body to heal by creating an optimal environment for it to do so. This involves modifying movements, postures and loads that promote a pain free state. This allows pain to settle and function to be restored.

Throughout the healing process, the musculature around the spine, aka the core, must be trained progressively. Initially loads will be low and some atrophy is inevitable. However, as the biological tissues load baring capacity improves, loads can be increased.

Once the healed tissues hit a certain point, heavier weights and cyclical loading, when applied correctly, can be used to build tissues beyond the previous state.

The aim of rehabilitation from injury, is to build a stronger, more resilient spine with movements, postures and load tolerance that avoids future injury.
