Kim is an excellent case manager and her support for our daughter and our family has been amazing throughout. She has ensured that we have as little to worry about as possible. Transition to adulthood is not something that begins at 18. For young people with complex disabilities, transition planning should start much earlier, often between the ages of 12 and 14.

Waiting until services are ending can mean:

• Limited time to prepare emotionally and practically
• Long waiting lists for adult services
• Gaps in therapy, education or healthcare provision
• Increased anxiety for young people and families
• Important opportunities being missed

Early planning allows time to consider:

• Education and vocational pathways
• Adult health services and specialist provision
• Social care and funding arrangements
• Housing and supported living options
• Developing independence skills gradually and safely

Transition is not a single event — it is a process.

Neuropsychology Isn’t Just “Talking Therapy”

When people hear “neuropsychology”, they often assume it means one-to-one therapy sessions.

But neuropsychology is about far more than conversations in a clinic room.

Neuropsychologists help us understand how a brain injury or neurological condition affects:

• Thinking and processing
• Behaviour and emotional regulation
• Relationships and family dynamics
• Risk, safeguarding and resilience within a care team

In complex paediatric cases, the stability of the care team is crucial.

Where family dynamics are strained, emotions are heightened, or safeguarding risks are emerging, the care team can come under significant pressure. Without the right support, this can lead to burnout, breakdown of relationships, or even loss of essential care provision.

Neuropsychology can provide:

• Brain-informed training and guidance for care teams
• Support before challenges escalate
• Clear behavioural strategies rooted in neurobiology
• Reflective space to strengthen professional boundaries and communication

Functional Neurological Disorders (FND) are conditions where people experience neurological symptoms such as weakness, movement difficulties, sensory changes or seizures, but without structural damage to the nervous system.

In FND, the brain and nervous system are not damaged, but they are not functioning as they should. This can be just as distressing and disabling as other neurological conditions.

FND can affect:
• Movement and coordination
• Sensation and strength
• Speech or vision
• Seizures and fatigue
