✨ The Role:
We are seeking a dedicated, experienced and compassionate Primary Care Practice Nurse to join our healthcare team. The successful candidate will be responsible for providing comprehensive medical care to patients across a wide range of health issues, promoting wellness, and managing chronic conditions. This role offers an opportunity to make a meaningful difference in patients' lives through expert clinical practice and personalised patient care. The position is suitable for qualified medical practitioners committed to delivering high-quality healthcare services.
This role will work closely with our newly appointed Enhanced Nurse focus on delivering high-quality, proactive care to patients with long-term conditions and supporting the PCN’s wider health inequality and population health initiatives.
You’ll work across PCN practices and within local communities, using data-driven approaches to identify need and target resources effectively.
