Every rehabilitation journey is unique — but some stay with you long after the sessions end. Maurice’s story is one of those.

When we first met him, he was determined to regain his independence but needed the right support, structure, and encouragement to get there. Through consistent therapy, clear goals, and a team that believed in his potential, Maurice rebuilt not just his mobility, but his confidence and daily rhythm.

What stands out most is not the clinical progress, but the human connection — the trust, the humour, the small wins that slowly became big milestones.

Stories like Maurice’s remind us why rehabilitation matters. It’s not just about exercises or equipment. It’s about dignity, purpose, and helping someone reclaim the parts of life that truly matter.

Grateful to Maurice and his family for allowing us to share a glimpse of his journey. And proud of the Therapies on Thames team for the compassion and expertise they bring to every home they walk into.

Rehabilitation changes lives — one person, one goal, one step at a time.

Transforming Care through Rehabilitation: Why Care Homes Partner with Therapies on Thames?

At Therapies on Thames, we believe that moving into a care home shouldn’t mean the end of physical progress. It should be the start of a supported journey toward maintaining independence and quality of life.
We work closely with care home managers and staff to bridge the gap in rehabilitation resources, providing expert Physiotherapy and Occupational Therapy directly to residents.

What We Offer Care Homes:
• Specialist Rehabilitation: Expert support for Stroke recovery, Parkinson’s, post-operative care (hip/knee replacements), and fall recovery.
• Mobility & Independence: Tailored exercise programs to improve strength, balance, and the ability to perform daily tasks like dressing and walking.
• Equipment & Seating Assessments: Professional guidance on wheelchairs, seating, and home adaptations to ensure resident safety and comfort.
• Staff Training: We empower care teams with training on manual handling, falls prevention, and postural care—helping your home meet CQC standards.

Why Choose Us?
•A Two-Pronged Impact: We focus on the resident’s physical health while simultaneously reducing the burden on care staff by increasing resident independence.
•Seamless Integration: We don't just "visit"—we become part of your multidisciplinary team, providing detailed reports and clear communication for families and GPs.
•Experience You Can Trust: Our therapists are HCPC registered and DBS checked, with deep expertise in elderly care across Berkshire, Oxfordshire and more in South East of England

Better physical health leads to better mental wellbeing. Let’s work together to help your residents live life to the fullest.

Rehabilitation Is 24/7—That’s Why Collaboration Matters

As physiotherapists and occupational therapists, we know that meaningful progress doesn’t happen in isolation. Rehab isn’t just about the one hour spent with a therapist—it’s about how those strategies are lived, practiced, and reinforced every day.

That’s why at Therapies on Thames - Rehabilitation Specialists , we work hand‑in‑hand with care staff, whether in home care or care homes, to achieve the best results for our clients.

• Embedding therapy into daily life:
Care staff help ensure exercises, mobility strategies, and adaptations become part of everyday routines.
• Spotting changes early:
Their close involvement means subtle shifts in health or independence are noticed quickly, allowing timely intervention.
• Creating continuity:
Collaboration ensures therapy goals are reinforced consistently, reducing risks like falls or hospital readmissions.
• Empowering independence:
Together, we build environments that encourage safe independence and dignity—24 hours a day, 7 days a week.

Rehab is not a one‑off appointment. It’s a continuous journey, and when therapists and care staff unite, we create a seamless circle of support that truly transforms lives.

👉 At Therapies on Thames - Rehabilitation Specialists , this partnership is at the heart of everything we do—because the best outcomes come from working together.

If you are a care agency or care home team looking for an understanding partner to support your clients with holistic rehabilitation, we’d love to connect. Please reach out—we’re here to help.

Rehabilitation, Close to Home for Our Elderly Loved Ones

At Therapies on Thames, we believe recovery should happen close to the people and places that matter most. For elderly residents, being near family and community is especially important.
That’s why we’ve partnered with Caring Homes to bring inpatient rehabilitation services directly into local communities. Our new Gloucestershire Rehabilitation Centre, based at WWhittington House Nursing Home will provide a safe, welcoming space where residents can begin their journey back to independence — surrounded by loved ones.
What this means for you and your family:
- No long journeys for specialist care
- Families can stay closely involved in recovery
- Expert therapists working together to support every step
- A warm, homely environment that blends care with comfort
- Faster, smoother transitions from hospital to community
- Helping the NHS too — by easing hospital pressures and supporting timely discharges
- Ongoing support at home — after inpatient care, our team continues rehabilitation at home, ensuring progress is maintained and independence grows
Because rehabilitation isn’t just about physical recovery — it’s about feeling supported, reassured, and connected every step of the way.

When Mr H was discharded from hospital to a Sunbury care home, after a fall, and he was was bed bound. Six weeks of focused physiotherapy later, he’s now getting out of bed independently and walking short distances with his 4-wheeled walker. Best of all, he’s looking forward to visiting his family in a couple of weeks — a moment that once felt out of reach. Progress like this is why we do what we do. 👏
If you’d like to explore how our physiotherapy team can support your residents with strength, mobility and confidence, we’d love to have a conversation. Please get in touch to find out more.👇

It was great to call in and see Louise at Cliveden Manor Care Home - Avery Collection last week. Our Physiotherapist Rachel, was working with a long term client and resident when I arrive, who told me all about how much Physiotherapy had impoved her mobility. 👏 These beautiful flowers in the reception reminded me that spring is on the way.

Communication: The Most Important Aspect of Safe and Timely Discharge,
When hospitals worry about rising length of stay (LOS), the single most important factor isn’t just capacity—it’s communication.

Patients and families often receive inconsistent messages—from A&E to the ward, and then again when discharge plans shift depending on clinical need. These changes are sometimes unavoidable, but without clear, consistent communication they create confusion, erode confidence, and delay safe discharge.

That’s why communication must be the foundation of every discharge plan:
Start early: Introduce discharge planning at admission.
Stay consistent: Align messaging across A&E, wards, therapy teams, and discharge coordinators.
Adapt transparently: When plans change, explain why and what support remains in place.
Provide one point of contact: Patients and families need a reliable voice to cut through complexity.

When communication is clear, consistent, and compassionate, patients and families feel reassured, delays reduce, and recovery continues in the right environment. Hospitals benefit too—shorter LOS, smoother patient flow, and safer transitions.

🌱 Shared goal: safe, positive, well‑coordinated transitions.
