The vermilion border (VB) is often described as a “barrier” to filler migration.
Ultrasound suggests otherwise.
The VB represents an epithelial transition zone – not a deep structural septum.
Migration appears to relate more to depth of placement than surface entry point, and is more frequently observed with vertical vector techniques.

I don’t think anyone enjoys challenging colleagues publicly.
But when medical claims are made with certainty, without evidence, and amplified to thousands of patients and practitioners, silence becomes a choice too.
This isn’t about ego or “being right”, but about responsibility to patients, to trainees, and to the profession.
We should be able to question claims without questioning character.

Which is the correct depth?
(A = vertical, B = cannula, C & D = horizontal)
In our recent ultrasound study (see previous post), filler was identified deeper than the usual intended subcutaneous plane, regardless of injection technique.
Greater needle visibility may suggest more superficial placement, but surface appearance alone is not a reliable indicator of true depth.
Image C is most likely to represent subcutaneous placement – subject to confirmation by ultrasound.

Very pleased to share that our study has now been published Online First in Plastic and Reconstructive Surgery (link in bio).
In this cross-sectional ultrasound study of 126 participants, we examined in vivo lip anatomy and hyaluronic acid filler behaviour in treated and untreated lips.
Key Findings:
• The subcutaneous layer of the lips averages less than 1mm thickness in both upper and lower lips, making true subcutaneous filler placement unreliable.
• Even when superficial techniques are intended, intramuscular (pars marginalis) placement is common.
• Vertical injection techniques were strongly associated with deeper and multilayer involvement, hypervascularity, and filler migration.
• Filler spread was common (62%) and markedly technique-dependent.
• Lip anatomy varied significantly with age, gender, ethnicity, BMI, and lip type.
Overall, these findings challenge the assumption that intended injection depth reliably reflects actual filler placement, and support a more individualised, anatomy-led and ultrasound-informed approach to lip filler treatment.

The New King of Hype: NAD+
Regenerative aesthetics follows a familiar pattern. Each cycle introduces a new concept with plausible biology, limited direct aesthetic evidence, and rapidly expanding claims. Over time, the language evolves faster than the outcomes.
This pyramid is ordered by degree of claim inflation – from mixed evidence at the base to increasingly abstract and extrapolated promises at the top.
PRP/PRF formed the foundation.
Everything above reflects growing distance between biological plausibility and clinical proof.
NAD+ is the current apex – it won’t be the last.
