How much time would I need to put aside?

Your first consultation can last up to 45 minutes, here your menopause specialist will discuss your full history, symptoms and try to understand your needs.

Your clinician will put a plan together taking a whole person approach, this might include medication, tests or lifestyle advice.

Follow-up consultations last up to 25 minutes. This usually provides sufficient time for women to explain any changes and hopefully improvements in their situation.

At the York Clinic, women and really listened to and given individual, specialist advice and choices regarding their treatment options.

Last night I had the opportunity to speak on Look North about the new Women’s Health Initiative in a conversation with Peter Levy that did not shy away from some of the more difficult realities facing women in healthcare today.

We discussed the broader impact of misogyny and stigma within the NHS and how this continues to affect women’s experiences, outcomes and access to support. These are not always comfortable conversations but they are necessary ones.

As a GP and women’s health specialist I see every day how gaps in understanding, funding and prioritisation can leave women feeling unheard or underserved. Change starts with acknowledging these issues openly and committing to doing better.

It is encouraging to see women’s health being given more visibility through initiatives like this but there is still a long way to go. We need sustained focus, education and cultural change across the system.

Grateful to have been part of the discussion and hopeful that it contributes to progress.

A menopause specialist is a healthcare professional who has additional knowledge and skills, assessing and treating women with complex needs such as multiple treatment failures, POI, complex medical problems, high risk cancer genes or hormone dependant cancer.

They accept referrals of more complex patients and support colleagues to manage patients with higher risk factors or where there are multiple factors that affect decision making.

Testosterone is an important hormone for many women, particularly during and after the menopause. Some women find that testosterone therapy can help improve libido, as well as boost mood, energy levels, and concentration.

Before considering testosterone, your clinician will first review or initiate hormone replacement therapy (HRT). It’s important to optimise oestrogen and progesterone treatment initially, as this provides the foundation for testosterone to work effectively.

If testosterone is prescribed, you will typically need an initial blood test, followed by regular monitoring to ensure the treatment is both safe and effective. These tests help guide dosing and track your response over time.

This week, EastEnders highlighted the challenges many women face when seeking help for menopause.

Menopause symptoms are real and can be deeply debilitating. Every woman deserves to be listened to, taken seriously, and supported with appropriate care.

Effective menopause treatment offers both immediate relief and long-term health benefits. While HRT is an option for many, treatment is not one-size-fits-all. Supporting and balancing hormone health is key to improving overall quality of life, and there are many ways to achieve this.

👏🏽 An appreciation post for the often overlooked Topical Oestrogen

After menopause, reduced oestrogen levels lead to changes in the vulval, vaginal, and pelvic tissues:

• Reduced blood flow – can make tissues thinner and less flexible
• Loss of collagen and elasticity – affects vaginal and vulval support
• Changes in pH and lubrication – can increase dryness, irritation, and risk of infections

These changes can impact pelvic floor function, sexual comfort, and overall urinary health.

🤍 Topical (local) oestrogen is one of the most effective and safe treatments available. It works by replacing oestrogen directly in the vaginal and urinary tissues, helping them become healthier, more flexible, and better lubricated.

🤍 Unlike systemic hormone replacement therapy (HRT) tablets or patches, topical oestrogen works mainly in the local area, with only very small amounts absorbed into the bloodstream.
