NEWBORN with ANENCEPHALY, a severe neural tube defect.

๐Ÿง  What is ANENCEPHALY?

๐Ÿ‘‰๐Ÿพ It occurs when the neural tube fails to close during early fetal development (around the 23rdโ€“26th day after conception).

๐Ÿ‘‰๐ŸพThis leads to absence of major portions of the brain, skull, and scalp.

๐Ÿ‘‰๐ŸพThe exposed neural tissue is often covered by a thin membrane, as seenon the top of the head in the image below.

โš ๏ธ CLINICAL FEATURES:

๐Ÿ‘๏ธInfants may be born with absent or rudimentary brain tissue.

๐Ÿ‘๏ธAssociated with abnormal facial features, low-set ears, and scalp/cranial defects.

๐Ÿ‘๏ธSadly, most newborns do not survive beyond a few hours or days after birth.

๐Ÿ”Ž RISK FACTORS:

๐ŸšฉFolate deficiency during pregnancy.

๐ŸšฉGenetic predisposition.

๐ŸšฉEnvironmental factors (maternal diabetes, certain medications, hyperthermia in early pregnancy).

โœ… PREVENTION:

๐Ÿซธ๐Ÿพ Sticking to Early antenatal schedule

๐Ÿซธ๐Ÿพ Folic acid supplementation (400 mcg daily before conception and during early pregnancy) has been proven to significantly reduce the risk of neural tube defects like anencephaly and spina bifida.

๐Ÿซธ๐Ÿพ A good nutrition

๐Ÿ“THE FOUR MAIN STAGES OF WOUND HEALING๐Ÿ“

๐Ÿฉธ 1) Hemostasis (stopping the bleeding) โ€” minutes to hours

๐Ÿ‘‰๐ŸพBlood vessels narrow to reduce bleeding

๐Ÿ‘‰๐ŸพPlatelets stick together and form a clot

๐Ÿ‘‰๐ŸพThe clot becomes a scab, which protects the wound

โœ…GOAL
Stop bleeding and seal the wound surface

๐Ÿ”ฅ 2) Inflammation (clean-up phase) โ€” 1 to 3 days

๐Ÿ‘‰๐ŸพArea becomes red, warm, swollen, and tender

๐Ÿ‘‰๐ŸพWhite blood cells arrive to:

๐Ÿ‘‰๐ŸพDestroy germs

๐Ÿ‘‰๐ŸพRemove dead tissue and debris

๐Ÿ‘‰๐ŸพYou might see clear or slightly yellow fluid โ€” that can be normal early on

โœ…GOAL
Prevent infection and clean the wound

๐ŸŒฑ 3) Proliferation (rebuilding tissue) โ€” days to weeks

๐Ÿ‘‰๐ŸพNew blood vessels grow (granulation tissue โ€” looks red and bumpy)

๐Ÿ‘‰๐ŸพCollagen forms to rebuild the skinโ€™s framework

๐Ÿ‘‰๐ŸพEdges of the wound pull together (contraction)

๐Ÿ‘‰๐ŸพNew skin cells (epithelium) grow over the wound

โœ… GOAL
Fill in and cover the wound

๐Ÿงต 4) Maturation/Remodeling (strengthening) โ€” weeks to months

๐Ÿ‘‰๐ŸพCollagen fibers reorganize and strengthen

๐Ÿ“Scar becomes:

๐Ÿ‘‰๐ŸพFlatter

๐Ÿ‘‰๐ŸพPaler

๐Ÿ‘‰๐ŸพSofter over time

๐Ÿ‘‰๐ŸพSkin never returns to 100% original strength, but gets close (80% โ€“ 90%)

โœ… GOAL
Makes the repaired tissue stronger and more flexible

The Minister for Health, Hon. Kwabena Mintah Akandoh, MP, has directed the Technical Advisor to the Ministry, Dr. Korku Awoonor, to constitute a technical working group to undertake a comprehensive audit of all health facilities in the Oti Region.

The audit will focus on assessing the structural integrity, operational capacity, and overall productivity of these facilities, with a view of strengthening service delivery and safeguarding patient safety.

The directive was issued during the Ministry of Healthโ€™s two-day management retreat and forms part of Governmentโ€™s broader agenda to improve oversight of health infrastructure and ensure that public health facilities across the country consistently meet approved safety, quality, and performance standards.

As part of complementary measures to enhance the quality of care and patient experience, the Health Minister has also ordered the introduction of 24-hour cleaning services in all public hospitals nationwide.

This intervention is aimed at improving hygiene standards, strengthening infection prevention and control, and creating safer, more patient-friendly healthcare environments.

Together, these initiatives are part of Ministryโ€™s commitment to raising standards across the health sector through proactive supervision, improved facility management, and sustained investments in quality healthcare delivery.

๐Ÿšจ COMPARTMENT SYNDROME๐Ÿšจ

A True Medical Emergency

Compartment syndrome occurs when pressure builds up inside a closed muscle compartment, cutting off blood flow and leading to muscle and nerve damage.

๐Ÿง‘โ€๐Ÿง’ TYPES

๐Ÿ”ด ACUTE cases often follow fractures, crush injuries, or tight casts and require urgent fasciotomy.

๐Ÿ”ต CHRONIC (exertional) type appears during exercise and improves with rest.

โš ๏ธ SIGNS AND SYMPTOMS

๐Ÿ‘‰๐ŸพREMEMBER the 6 Pโ€™s:

โ™จ๏ธPain (out of proportion),
โ™จ๏ธ Pallor,

๐Ÿฉบ THE SHARED PLACENTA ๐Ÿฉบ

This phenomenon inappens in twins formation especially in identical twins when both share one placenta.

โš ๏ธ RISKS:
There are some risks associated with the phenomenon even though itโ€™s a shared placenta:

1. Uneven nutrient
2. Uneven blood supply โ†’ may cause Twin-to-Twin Transfusion Syndrome (TTTS)

๐Ÿฉบ CARE:
It requires frequent monitoring to keep both babies healthy.

โœจ MANAGEMENT:
Though it is common in identical twins but manageable with close medical supervision!

๐ŸงDIFFERENCE BETWEEN VENOMOUS and NON-VENOMOUS SNAKES๐Ÿ

1๏ธโƒฃ BITE MARKS ๐Ÿ
Non-venomous snakes usually leave several small, evenly spaced tooth marks. Venomous snakes often leave two deeper fang punctures, although this can vary depending on the species and the bite angle.

2๏ธโƒฃ BEHAVIOR ๐Ÿ
Venomous snakes are often more defensive when threatened and may hiss, coil, or strike to protect themselves. Non-venomous snakes are more likely to flee or rely on camouflage rather than confrontation.

3๏ธโƒฃ HEAD SHAPE ๐Ÿ
Many venomous snakes have a broader, triangular-shaped head due to venom glands, while many non-venomous snakes have a more rounded or narrow head. This feature is not always reliable, as some non-venomous species can flatten their heads to appear larger.

4๏ธโƒฃ PUPILS ๐Ÿ
In some regions, venomous snakes tend to have vertical, slit-shaped pupils, while non-venomous snakes often have round pupils. Lighting conditions and species differences can affect this trait.

5๏ธโƒฃ BODY and MOVEMENT ๐Ÿ
Venomous snakes may have thicker bodies and slower, more deliberate movements, while non-venomous snakes are often slimmer and faster. This is a general observation and not a strict rule.

๐ŸšจThough identification can be difficult and risky but if you encounter a snake or suspect a bite, keep your distance and contact trained professionals for safe handling and guidance.

๐Ÿšจ ABNORMALITIES OF THE NAIL PLATE ๐Ÿšจ

1. Terryโ€™s Nails

โ†’ Nail plate appears diffusely white or pale with a narrow distal pink or brown band
โ†’ Due to decreased vascularity of nail bed
โ†’ Common associations
โ†’ Hypoalbuminemia
โ†’ Liver cirrhosis
โ†’ Congestive heart failure
โ†’ Diabetes mellitus
โ†’ Chronic kidney disease

2. Salmon Patch (Oil Drop Sign)

โ†’ Yellow-pink translucent patch visible beneath nail plate
โ†’ Represents psoriatic involvement of nail bed
โ†’ Highly specific for nail psoriasis
โ†’ Often associated with
โ†’ Pitting
โ†’ Onycholysis
โ†’ Subungual hyperkeratosis

3. Splinter Hemorrhages

โ†’ Thin, longitudinal reddish-brown streaks under nail plate
โ†’ Caused by rupture of nail bed capillaries
โ†’ Seen in
โ†’ Trauma (most common)
โ†’ Infective endocarditis
โ†’ Vasculitis
โ†’ Antiphospholipid syndrome

4. Subungual Pigmentation (Low-Grade Hematoma)

โ†’ Brownish or reddish discoloration beneath nail plate
โ†’ Usually due to minor repeated trauma
โ†’ Pigment does not involve nail matrix
โ†’ Lesion grows out with nail growth
โ†’ Important differential: melanoma

5. Subungual Hematoma

โ†’ Dark red, maroon, or black discoloration
โ†’ Represents acute bleeding under nail plate
โ†’ Usually painful initially
โ†’ Common after blunt trauma
โ†’ Color migrates distally with nail growth

6. Green Nail Syndrome

โ†’ Green-black discoloration of nail plate
โ†’ Caused by Pseudomonas aeruginosa infection
โ†’ Occurs in moist environments
โ†’ Seen in
โ†’ Onycholysis
โ†’ Chronic water exposure
โ†’ Artificial nails

7. Leukonychia

โ†’ White spots or transverse white streaks on nail plate
โ†’ Due to abnormal keratinization of nail matrix
โ†’ Most commonly from minor trauma
โ†’ Lesions move distally with nail growth
โ†’ Not due to calcium deficiency

8. Blue Nail Discoloration

โ†’ Diffuse bluish coloration of nail plate
โ†’ Caused by reduced oxygenation or altered hemoglobin
โ†’ Seen in
โ†’ Cyanosis
โ†’ Methemoglobinemia
โ†’ Drug-induced (e.g., antimalarials, amiodarone)
โ†’ Advanced systemic illness

9. Yellow Nail Syndrome

โ†’ Yellow, thickened, slow-growing nails
โ†’ Loss of cuticle often seen
โ†’ Classic triad
โ†’ Yellow nails
โ†’ Lymphedema
โ†’ Respiratory disease (chronic sinusitis, bronchiectasis, pleural effusion)

10. Longitudinal Melanonychia

โ†’ Single or multiple brown-black vertical bands
โ†’ Pigment originates from nail matrix melanocytes
โ†’ Common causes
โ†’ Benign melanocytic activation
โ†’ Junctional nevus
โ†’ Physiological (especially in darker skin types)
โ†’ Requires differentiation from melanoma

11. Subungual Malignant Melanoma (Early)

โ†’ Irregular pigmented band with
โ†’ Uneven borders
โ†’ Color variation
โ†’ Widening proximally
โ†’ Hutchinson sign may be present
โ†’ High suspicion if single digit involved
โ†’ Does not move with nail growth

12. Subungual Malignant Melanoma (Advanced)

โ†’ Large irregular pigmented mass
โ†’ Nail plate destruction or lifting
โ†’ Possible ulceration or bleeding
โ†’ Represents late presentation
โ†’ Poor prognosis if diagnosis delayed
