ESD of a large re**al LST-granular with SMI. Photos illustrate incision, dissection across large submucosal vessels, "trimming', gradual mobilization of the lesion, use of red dichromating imaging(RDI) for localization of bleeding and hemostasis, dissection below SMI, the final rather large ESD defect and the lesion post-dissection and pre-fixation with pins/needles.

Acute bleeding originating from a gastroesophageal type 1 (GOV-1) varix. Endoscopic variceal band ligation performed with first two bands placed on and just proximal to the spurting site. Additional bands placed on different variceal columns and sites. Photos illustrate site of bleeding with evident spurting and succesful rubber band placement (forward and retroflex view).

A busy yet interesting week: A**l ca**l high-grade dysplasia treated with ESD. Photos depict a. the lesion prior to resection b. optimal use of saline immersion and cap during pulsed cut and submucosal dissection stages of ESD c. Cutting alongside Morgagni columns d. the abundance of vessels some of which corresponding to regional vascular plexus e. part of the dissection was perpendicular to the internal a**l sphincter fibers f. Final cut g. full mobilization of the lesion h. electrocautery at the dissection site/ESD defect. Saline, 1.5mm Dual J knife, short ST hood, 4mm Coagrasper, ESG 300 (PulseCut slow and forced coag) were used.

Πρώτη χρήση στην Ελλάδα του νέου συστήματος συρραφής Sutuart της Olympus για σύγκλειση εμμένοντος συριγγίου μεταξύ ορθού και ουροδόχου κύστης σε ασθενή με Ca ουροποιητικού και χειρ/κή εκτομή για Ca παχέος. Στις εικόνες φαίνονται: α. η είσοδος καθετήρα εντός του συριγγίου β. ο καυτηριασμός του στομίου και συριγγώδους πόρου από δύο διαφορετικές θέσεις με χρήση του additional working channel (AWC) της Ovesco γ. η είσοδος του ειδικού ράμματος V-LOC δια του πρωκτού με σύλληψη από το ράμμα (όχι από τη βελόνα) δ. η είσοδος της βελόνας και του ράμματος εντός του τοιχώματος δια του χάσματος του συριγγίου και η έξοδος της βελόνας ε. περιστροφή της βελόνας στ. πιο αποτελεσματική σύσφιξη του ράμματος/σύγκλειση χάρη στη χρήση 2 εργαλείων (Sutuart από κανάλι εργασίας ενδοσκοπίου και λαβίδα από AWC).
First use in Greece of the new Sutuart device to provide closure of a rectovesical fistula in an urinary tract Ca patient post-surgical resection for re**al Ca. Still frames show a. catheter inside the fistula tract b. cauterization of fistula or***ce and tract using endoscope's working channel and the additional working channel (AWC) by Ovesco c. introduction of the V-LOC suture inside the re**um while grasping the suture (not the needle) to allow free movement thus avoiding pinching/anchoring d. the needle pinching through the wall, fistula tact and finally rotating back into the lumen e. needle rotation f. effective tightening using two intruments, Sutuart (through the scope's working channel) and forceps (through the AWC).

The elusive nature of early gastric cancer. Even in HD systems such lesions can escape diagnosis as they may show a subtle "reddish" appearance under white light (kudos Dr Koukouratos from General Hospital of Larisa for detecting and getting reliable histology at a single bite). Narrow band imaging and indigo carmine chromoendoscopy are useful tools for detection and demarcation of these lesions. The lesion was removed using a classic/old school ESD approach. Marking, submucosal injection, circumferential incision (pulse cut slow effect 2 120W) and dissection (forced coag effect 2 45W) were performed using Dual j knife and a short ST hood. Exposed vessels were treated with Coagrasper 5mm (soft coag effect 4 70W). ESD defect was closed using 16mm hemoclips. ESD team A. Manolakis and Tr.Mpektsis (who also performed final cuts thus releasing the "suspended" lesion from the submucosa). Many thanx to our Anesthesiology team. Patient discharged two days later.
Μεγάλη εβδομάδα με ESD για πρώιμο γαστρικό Ca για το Department of Gastroenterology, University of Thessaly School of Medicine. Οι φωτο αναδεικνύουν τη δυσχέρεια στην αναγνώριση τέτοιων πρώιμων βλαβών και την αξία της χρήσης εικονικής αλλά και κλασικής με Indigo carmine χρωμοενδοσκόπησης για τον εντοπισμό και οριοθέτηση τέτοιων βλαβών. Οι λοιπές φωτό απεικονίζουν στάδια από την ESD. Χρήση short ST hood, oριοθέτηση/σήμανση και διατομή με Dual j knife, καυτηριασμός αγγείων με Coagrasper 5mm, σύγκλειση με clips 16mm. En bloc αφαίρεση-καθήλωση δείγματος με Insulin needles σε nylon-coated cardboard.

Patient with Pancreatic Ca presents with massive bleeding from a large vessel at the duodenum. Within the video, pulsatile oozing, inability to fully control bleeding with epinephrine injection and initial cauterization can be seen. Combination of aggressive electrocautery using coagrasper 5mm and deployment of a padlock OTSC (less effective in GI hemorrhage compared to ovesco OTSC) led to successful management of vessel(s) while preventing perforation-related adverse outcome. Patient discharged.

Spontaneous bleeding in a patient with J-pouch (colectomy+ileal pouch-a**l anastomosis). Oozing from a vessel located on the verge between ileal limbs. Hemostasis using diluted epinephrine and 2 hemostatic clips.

Το Ινστιτούτο Κλινικής & Επεμβατικής Γαστρεντερολογίας (Ι.Κ.Ε.Γ.) και η Γαστρεντερολογική Κλινική της Ιατρικής Σχολής του Πανεπιστημίου Θεσσαλίας, διοργανώνουν την Εκπαιδευτική Ημερίδα με θέμα "Σύγχρονοι προβληματισμοί στα Ιδιοπαθή Φλεγμονώδη Νοσήματα του Εντέρου". Επιστημονικός Υπεύθυνος είναι ο Καθηγητής Παθολογίας - Γαστρεντερολογίας Ανδρέας Καψωριτακης. Η Ημερίδα θα πραγματοποιηθεί το Σάββατο 2 Δεκεμβρίου 2023 στο Αμφιθέατρο Ιπποκράτης στην Ιατρική Σχολή, στη Λάρισα.
