Impotence – a common but treatable condition ⚕️

Erectile dysfunction, popularly known as impotence, is the constant or recurrent inability to achieve or maintain an er****on sufficient for satisfactory s*xual in*******se. It is one of the most common male s*xual disorders, especially after the age of 40 📊.

Research shows that approximately 1 in 2 men over the age of 40 experience some degree of erectile dysfunction, and the risk increases with age and the presence of other associated diseases 📈.

The most common causes are:
🫀 cardiovascular diseases and poor circulation
🍬 diabetes
🔴 high blood pressure
⚖️ hormonal disorders, especially low testosterone levels
🧠 neurological diseases
🚬 to***co, alcohol or drug use
😟 mental stress, anxiety and depression

Erectile dysfunction can be an early sign of more serious systemic diseases, such as coronary heart disease ❤️. Investigating the cause is essential for choosing the right treatment. Currently, medicine offers effective solutions, personalized according to each patient 💊👨‍⚕️.

👇Er****on and male fertility – two different, but often confused aspects

Within the andrological practice, we frequently encounter men who associate the ability to have an er****on with fertility. It is important to emphasize that er****on and fertility are two distinct functions, regulated by different mechanisms.

✔️ Er****on is a vascular and neurogenic phenomenon, influenced by hormonal, psychological factors and cardiovascular health

✔️ Male fertility depends mainly on the s***m quality – concentration, motility and normal morphology of the s***matozoa

‼️ Thus, a man can have an active and normal s*x life, but still be infertile. Likewise, a man with erectile dysfunction can have normal s***matogenesis and become a father through natural or assisted methods.

📊 Recent studies show that approximately 15% of the infertile couples are faced with a cause of male origin, and of this percentage, a significant part have abnormal s***mograms despite intact s*xual function.

🔬 The evaluation of male fertility requires specific analyses: s***mogram, s***m culture, hormonal tests and, sometimes, genetic or imaging investigations.

👉 If you experience difficulties in achieving a pregnancy, it is important to consult an andrologist, even if s*xual function seems normal.

Has male fertility declined globally or are we just diagnosing it better now?

🧪 The answer is supported by scientific data: male fertility is in a real, measurable and worrying decline.

📉 A study published in 2022 (Levine et al.) analyzed data from over 50 countries and concluded that:

➡️ S***m concentration (s***m count/ml) decreased by over 50% between 1973 and 2018
➡️ The rate of decline accelerated after 2000

⚠️ This phenomenon is not just due to advances in diagnosis. We are dealing with a real decrease in s***m parameters: total count, motility, morphology.

📌 Main factors identified:
- Pollutants with hormonal action (pesticides, plasticizers – BPA, phthalates)
- Chronic stress and lack of sleep
- Ultra-processed food and obesity
- Smoking, alcohol, drugs
- Sedentary lifestyle and thermal exposure (sauna, laptop on the abdomen, etc.)

📊 In parallel, it is true that andrological investigations are today more accessible and more precise:
✔️ High-resolution testicular ultrasound
✔️ Specific hormonal tests
✔️ Genetic and s***m DNA fragmentation analyses
✔️ Modern testicular biopsies (micro-TESE)

🧬 Conclusion: male fertility is affected both quantitatively and qualitatively and this is objectified by solid international data. It is not just an impression.

👨‍⚕️ Preventive andrological evaluation is essential, even in the absence of an immediate desire to conceive. Fertility is NOT a permanent given‼️

Chronic Stress and Male Fertility: A Scientifically Proven Link ⚠️

Prolonged psychological stress has a significant impact on male reproductive function. Studies show that men exposed to chronic stress can experience low testosterone levels, decreased libido, and significant alterations in s***m quality. 🧬

📊 A study published in Fertility and Sterility magazine found that men with high levels of stress had s***m concentration up to 38% lower and motility reduced by 31%, compared to those without exposure to intense stress.

🧪 A research from the American Journal of Men's Health also showed that men with high levels of stress are 2 times more likely to have increased s***m DNA fragmentation – a factor associated with low fertilization rates and an increased risk of miscarriage.

📉 Another study from Denmark, on a sample of over 1,200 men, showed that those with increased stress levels were 15% more likely to present abnormal values of s***m parameters, including morphology and seminal volume.

👉Understanding the impact that stress has on the male reproductive health remains essential in the evaluation of male infertility, even in cases without obvious urological pathologies. 🔍

Teratozoos***mia and male infertility

Teratozoos***mia is a condition characterized by an increased percentage of s***m with abnormal morphology. According to the criteria of the World Health Organization (WHO), a normal percentage of s***m with typical shapes should exceed 4%. The presence of a significant proportion with abnormal shape can affect the fertilization of the oocyte and, implicitly, the chances of achieving a pregnancy.

🔬 Possible causes:
The etiology of teratozoos***mia is multifactorial and may include:
- genetic factors (chromosomal abnormalities, mutations of genes involved in s***matogenesis)
- hormonal disorders (testosterone deficiency, hyperprolactinemia, hypothalamic-pituitary-testicular axis dysfunction)
- chronic genitourinary infections
- varicocele (one of the most common correctable causes)
- exposure to toxins (smoking, alcohol, drugs, pesticides, radiation, elevated testicular temperatures)
- lifestyle factors (unbalanced diet, oxidative stress, lack of sleep)

📈 Evaluation and diagnosis:
The diagnosis of teratozoos***mia is established following a correctly performed s***mogram, under standardized laboratory conditions. In some cases, additional investigations are necessary, such as s***m DNA fragmentation testing, hormonal evaluation or testicular ultrasound.

🩺 Therapeutic options:
Treatment is individualized depending on the identified cause and the severity of the condition. It may include:

▪️lifestyle changes and avoidance of harmful factors
▪️antioxidant supplementation (vitamin C, E, zinc, L-carnitine, coenzyme Q10, etc.)
▪️hormonal treatments
▪️surgical correction of varicocele
▪️use of assisted reproductive techniques (IUI, IVF, ICSI), depending on the clinical context

📌 It is essential that the evaluation of the male infertility to be performed by a urologist with expertise in andrology to determine the exact cause and to institute the appropriate treatment.

Polys***mia – when ej*******on exceeds normal values

Polys***mia is an increase in the ej*****te volume above 5.5 ml, a rarer condition, but which can have multiple causes.

This can be the result of prolonged abstinence ⏳, s*xual hyperstimulation 🔥 or, in some cases, it can be associated with seminal vesicle, prostate or hormonal imbalances ⚖️.

Although most often it does not require treatment, persistent polys***mia or accompanied by other symptoms (pain, discomfort, changes in consistency or color) may signal an underlying condition that requires medical evaluation 🩺.

The treatment is aimed at the identified cause and it may include hormonal adjustments, anti-inflammatory drugs, antibiotics 💊 or, in rare cases, specific interventions on the male reproductive system.

Monitoring these changes and correct clinical evaluation can make a difference in the early identification of the possible pathologies.

🔹 Parvis***mia – What does it mean and why is it important not to ignore this symptom?

Parvis***mia is the presence of a reduced volume of s***m during ej*******on, generally below 1.5 ml (according to the reference values established by the World Health Organization). Although it is not always associated with a serious condition, it can indicate problems that deserve investigation.

📌 The causes of parvis***mia can be multiple, including:

🔹Seminal vesicle dysfunction (hypoplasia or obstruction)
🔹Prostate diseases (chronic prostatitis, sclerosis)
🔹Retrograde ej*******on (s***m reaches the bladder instead of being eliminated)
🔹Hormonal imbalances (low testosterone levels)
🔹Trauma, surgery or conge***al anomalies
🔹Infections of the reproductive tract

📉 In some cases, parvis***mia may be associated with male infertility, and in others it may be just a temporary manifestation caused by factors such as dehydration or frequent s*xual activity.

👨‍⚕️ It is important to consult an urologist if you notice a persistent decrease in the ej*****te volume or other associated symptoms (pain, discomfort, changes in the consistency or color of semen).
Through investigations such as s***mogram, ultrasound of the male ge***al tract and hormonal analyses, we can quickly identify the cause and establish an effective treatment plan.

🩺 The role of the seminal vesicles and ejaculatory ducts in the male reproductive system

The seminal vesicles and the ejaculatory ducts are important components of the male reproductive system, having essential functions in the process of formation and elimination of seminal fluid.

🔬 The seminal vesicles are paired glands, located posterior to the urinary bladder, which contribute significantly to the total volume of the semen. They secrete an alkaline fluid, rich in fructose, prostaglandins and other substances that support the mobility and the viability of s***m. Fructose is the main source of energy for s***m, and the alkaline environment helps neutralize vaginal acidity, thus favoring fertilization.

🔬 The ejaculatory ducts are short channels, formed by the union of the vas deferens with the seminal vesicle duct, which cross the prostate and open at the level of the prostatic urethra. Their role is to transport both s***m from the testicles and the secretions of the seminal vesicles during ej*******on.
Together, these structures contribute to ensuring male fertility by facilitating the formation of an optimal environment for the transport and survival of s***m.

📅 For the evaluation of the reproductive function or in the event of any urological symptoms, a specialist consultation is recommended.

🔍 Circumcision and male fertility 🤔

Circumcision is a surgical intervention that removes the fo****in (the skin that covers the g***s of the p***s) 🩺. Although it is often associated with hygiene or tradition reasons, the question often arises: does it affect fertility? 👶

The answer is simple: it does NOT directly affect male fertility ❌. S***m quality, hormone levels and erectile function remain unchanged ✅.

However, circumcision can bring indirect benefits 💡. It reduces the risk of urinary infections, local inflammations and certain s*xually transmitted diseases 🦠. These conditions, if frequent or untreated, can influence the health of the reproductive system over time.

👉 Therefore, circumcision is not a solution for infertility, but it can help prevent problems that could affect fertility in the long term.

👇Azoos***mia: Causes and Treatment Options

Azoos***mia is a condition in which s***m are completely absent from the seminal fluid, being a major cause of male infertility. There are two main types:

🔹 Obstructive azoos***mia – it occurs when there is a blockage in the s***m pathway, although the testicles produce normal s***m.
✅ Treatment: in many cases, it can be corrected by surgery to unblock the ducts or by harvesting s***m directly from the testicle for in vitro fertilization.

🔹 Non-obstructive azoos***mia – it occurs when the testicles do not produce enough s***m or at all, due to hormonal, genetic or medical problems.
✅ Treatment: options include hormone therapy, specific supplements, lifestyle changes and, in some cases, direct s***m collection for assisted reproduction.

👨‍⚕️ A correct diagnosis, made by an urologist/andrologist, is essential to find the best personalized solution.

If you are facing fertility problems, don't hesitate to schedule a medical consultation!

Fertility and s*xual contact: what you need to lnow?

🤔 How often should you have s*xual contact to get pregnant?
👉 Specialists recommend regular s*xual contact, 2-3 times a week, and during ovulation (approximately days 12-16 of the menstrual cycle) even daily or every other day. This rhythm ensures a constant presence of s***m in the female reproductive tract, increasing the chances of conception.

💡 Does delaying ej*******on affect male fertility?
🔹 There is no scientific evidence to support that delaying ej*******on improves fertility.
🔹 On the contrary, regular ej*******on helps maintain s***m health. Too long a period of abstinence can lead to the accumulation of less mobile s***m and a decrease in s***m quality.

🔬 If you are facing difficulties in getting pregnant, an urological and andrological consultation can offer you personalized solutions.
