As it's Alcohol Awareness Week, today's is Alcohol and Breastfeeding.

Did you know? You don’t need to express breastmilk to remove alcohol from your system - it leaves your milk as it leaves your bloodstream.

You can have the occasional small drink while breastfeeding, but regular or heavy drinking (binge drinking) requires extra care to reduce your baby’s exposure.

Before drinking, think - who is the sober, responsible adult caring for your baby?

Some important reminders:

⭐If you’ve had any alcohol, do not bed share or fall asleep while holding your baby.

⭐If you’ve been drinking heavily, your baby should be cared for by someone who hasn’t had any alcohol.

⭐If you feel unwell, disoriented, or have vomited, you should not care for your baby alone. Make sure there’s a sober adult with your child.

⭐You can express for comfort and to maintain your supply - but it’s not necessary to “pump and dump” to remove alcohol from your breastmilk.

What IS Colic?

Colic is when a healthy baby cries intensely for more than 3 hours a day, 3+ days a week, for over 3 weeks – often in the evening or at night. It affects up to 1 in 4 infants, usually starting around 2–4 weeks old and peaking at 6 weeks. The good news? It typically resolves by 3–4 months!

Signs Your Baby Might Have Colic:
- 😢 Inconsolable crying (like screaming or pain)
- 👶 Body tension: Clenched fists, arched back, legs pulled to tummy
- 🔴 Flushed face, bloated belly, or frequent gas

Why Does It Happen?
The exact cause is unknown, but theories include:
- 🌱 Immature digestive or nervous systems
- 🥛 Food sensitivities (e.g., cow’s milk protein in breast milk or formula)
- 🌪️ Gas from swallowed air during crying
- 🧠 Sensory overload or early migraines

Soothing Strategies to Try:
1️⃣ Comfort Tactics: Swaddle, rock, use white noise (fan/vacuum), or offer a pacifier.
2️⃣ Feeding Adjustments:
- Breastfeeding? Temporarily avoid caffeine, dairy, or gas-producing foods (e.g., cabbage, onions).
- Formula-feeding? Try a hypoallergenic option.
3️⃣ Movement: Car rides, stroller walks, or baby wearing in a sling.
4️⃣ Tummy Time: Gently massage baby’s belly, lay them across your lap or try the 'Tiger in the Tree' hold

→ Note: Avoid gripe water/probiotics and other 'medications' – evidence is weak! - More about this in a future post!

When to Call the Doctor:
Seek help if your baby:
- 🚨 Has fever, vomiting, bloody stools, or trouble breathing.
- 📉 Isn’t gaining weight or has crying spells past 4 months old.
- 😩 You’re overwhelmed – support is essential!

Parents: Take Care of YOU Too!
Colic is NOT your fault, and it’s okay to feel exhausted. To avoid burnout:
- 🔁 Take shifts with a partner/friend.
- 🛑 If overwhelmed, place baby safely in their crib and step away for 5–10 minutes.
- 📞 Reach out to a helpline (e.g., Cry-sis: 0800 448 0737 - UK charity) or your midwife.
Remember: Shaking a baby can cause brain damage – always ask for help when needed.*

Nexus Private
This package shifts the balance of care to being more obstetric led. You still see a Midwife and get the classes (minus the public hospital workshop) and postnatal home visits. However, this package is designed to balance midwifery and obstetric care for those clients giving birth in the private maternity system.
Your obstetrician is your lead healthcare Provider, and you’ll have around 6 check-ups with them, and another 3 or 4 with your Midwife. You’ll have plenty of time to ask questions during your appointments and the midwives can add a valuable Midwifery perspective. Your Midwife will then see you at home for your postnatal home visits, alongside your Obstetrician follow-ups.

Prime Private
This is a solely private package, designed for those clients wishing to have all their care with an Obstetrician and who want to give birth in the private healthcare system. You can add on the Midwife-led antenatal classes and postnatal home visits for completeness, to ensure you get the best possible pre-birth education and at-home postnatal lactation and newborn care support.

Generally, we prefer to start your package around the time you have your 12-week screening ultrasound and blood tests. However, we welcome you to see us prior to this.
If you’d like to come and have an early pregnancy ultrasound scan with one of our midwives, we can date the pregnancy for you, give you a pregnancy confirmation letter, as well as give you essential early pregnancy information.

Nexus Public
Our Nexus Package was developed with the help of one of our now retired Obstetricians (OBs) to cater for people wanting a bit more than the Best of Both experience.
The package comprises of mostly Midwife check-ups, with the OBs doing the anomaly ultrasound scan and overseeing (and advising) on screening, blood tests, and growth ultrasounds. We would expect you to register with the Public Hospital of your choice between 16-24 weeks, (I usually advise after 20) and then check in with them again at 37 weeks and again at 40 weeks, if you haven’t given birth by then. This is the perfect combination of midwifery-led care, access to tests and scans without the longer HA wait times and balancing that with the considerably cheaper HA birth.
Of course, this package also includes your antenatal class, public hospital workshop, Breathing and Relaxation workshop, and postnatal home visits.

Private vs Public Midwifery care: Where to Begin and which of our packages is right for you

Following on from my previous post about the HK Public and Private Systems and Annerley’s place supporting clients going through either, I thought you’d like to know more about out maternity packages.

At OT&P we have 4 maternity packages
Best of Both (our original package), that combines private Midwifery care with a public hospital birth.
Nexus Public, which combines private Midwife and obstetric care, along with a public hospital birth.
Nexus Private combines obstetric and Midwifery care with a private hospital birth.
And Prime Private, which is solely private obstetric-led care with a private birth.

Let’s explore these options a bit further:
The packages (part 1)
Our original Best of Both package was designed for those clients wanting more than the public system has to offer, without huge expenditure.
You will see our midwives for at least half of your regular checkups and the public Health Authority (HA) for the other half.
We arrange your appointments so that you do the higher-priced check-ups, such as the ones where you get blood test, injections, and anomaly ultrasound, etc., in the HA. In the HA there are often long waiting times and then a very short time with your clinician, (who might be a different one every time). Coming to Annerley for the regular midwifery antenatal check-ups means you can have considerably shorter waiting times, (personally, I am rarely late starting my consultations), and your dedicated Midwife, who will be with you through the whole journey, will make each appointment at least 30 minutes long, giving ample time to ask questions.
In addition, you’ll likely see your own Midwife for your antenatal class, which is also a part of the package for first time parents, and you’ll see your Midwife for your post-natal home visits.
Sue will guide you through what to commonly expect when you go to the hospital for the birth of your baby during your Public Hospital Workshop and give you some relaxation and breathing tools during your Breathing and Relaxation class.
This is our cheapest package and can also be tailored for second (or more) time parents, because we know you won’t be needing all the classes.

Why Hong Kong’s Public System Might Surprise You
Public hospitals deliver over 40% of Hong Kong’s babies and excel where it matters most: safety, efficiency, and affordability . Here’s what makes them a compelling choice:

- Unbeatable Cost for Residents:
With a HKID card, vaginal delivery costs just HKD $300–$1,500, covering everything from the delivery to a 3+-day stay. Even emergency C-sections or NICU care won’t add to your bill. Compare this to private hospitals, where packages can surge to HKD $300,000 for complications.

- Emergency Excellence:
Public hospitals like Queen Mary (Hong Kong Island) and Prince of Wales (Shatin) have 24/7 obstetricians, anaesthesiologists, and Level III NICUs. Private hospitals may have to transfer critical cases to these facilities, separating mother and baby in emergencies . If you have a high-risk pregnancy, public is the safest bet.

- Natural Birth Advocacy:
C-section rates in public hospitals (~25%) are half those in private ones (~50%) . Unless medically necessary, natural birth is encouraged, with midwives guiding deliveries and immediate skin-to-skin contact prioritized.

- Guaranteed Admission:
Public hospitals cannot turn you away—even without a prior booking. In private hospitals, pre-paid deposits don’t guarantee a bed if wards are full on your delivery day.

Trade-offs? Costs can spiral with complications, emergency transfers to public NICUs disrupt bonding, and deposits (HKD $10,000–$20,000) lock you in early.
