【中醫痛症治療 Traditional Chinese Medicine Pain Treatment】
中醫治療疼痛的機理，可分為：
一、不通則痛：
出自李東垣 < 醫學發明 >，是指風、寒、濕、熱等外邪所致各類痹症，或筋出槽骨錯縫、骨架翻旋造成肌肉及筋膜攣縮僵硬，或者周身氣血混亂或痰凝氣滯血瘀，造成各種酸痛脹麻。
二、不榮則痛：
《素問臟氣法時論》泛指虛症導致的疼痛，可能包含骨鬆、血虛、飲食勞倦、脾胃虛弱、肝腎陰虛、陽虛水泛等所導致肌肉關節疼痛、無力、冷痛的現象。
三、情緒影響：
《素問•至真要大論篇》談到「諸痛癢瘡，皆屬於心」，中醫所說任物者謂之心 (任物：接受外界信息而作出反應)，因此情緒或心理因素也可能產生疼痛，例如纖維肌痛症，一種廣泛性的肌肉痛與深部組織疼痛，但也同時容易伴隨疲憊與睡眠、記憶及情緒問題，如憂鬱、焦慮等情緒障礙，以及認知功能障礙。
以中醫來說，疼痛屬於氣滯血瘀，針灸可以達到活血化瘀、疏通經絡效果，有效治療疼痛。亦有配合手法推拿、內服/外敷中藥、拔罐、艾灸、電針等治療。
The mechanisms of TCM in treating pain can be categorized into three main types:
1. Pain Due to Obstruction
This concept originates from Li Dongyuan's "Medical Inventions" and refers to various types of pain caused by external factors such as wind, cold, dampness, and heat. It also includes pain due to muscle and fascia tension, misalignment of bones, or disruptions in the body's Qi and blood flow, leading to symptoms like soreness, swelling, and numbness.
2. Pain Due to Deficiency:
As mentioned in "Su Wen: Discussion on the Laws of the Internal Organs," this type of pain is generally caused by deficiencies in the body. It may include conditions like osteoporosis, blood deficiency, fatigue, spleen and stomach weakness, liver and kidney yin deficiency, and yang deficiency, leading to muscle and joint pain, weakness, and cold pain.
3. Emotional Influence
According to "Su Wen: The True Doctrine of the Golden Chamber," emotions and psychological factors can also cause pain. For example, fibromyalgia, a condition characterized by widespread muscle pain and deep tissue pain, is often accompanied by fatigue, sleep disturbances, memory issues, and emotional problems such as depression and anxiety.
In TCM, pain is considered to be caused by Qi stagnation and Blood stasis. Acupuncture can effectively treat pain by promoting blood circulation, removing stasis and clearing meridians. Other treatments include massage, internal and external herbal medicine, cupping, moxibustion, and electroacupuncture.
