👔我頸痛, 做乜醫我隻腳

身體代償機制 (Body Compensation)
- 若我們有上身痛症(尤其長期慢性)如頭痛、頸痛、肩痛、背痛,身體有機會用其他位置代償
例如(代償由上到下):
* 頭/上頸前突 ->
* 脊椎變直 ->
* 減少生理弧度 ->
* 盆骨後傾 ->
* 髖關節外旋 ->
* 腳掌負重集中在外側
- 若要長期、有效的去治療根本原因,處理頸部 外,亦要改善下肢和腳掌的情況

上半身和下半身相互連結
- 例如上肢發力:在不同情況下,身體會選擇分別運用同邊或是另一邊的腳,作為重心腳(支點)
*用錯重心腳的話,上肢的發力效率會下降
- 效率下降,會導致力量&控制力同時下降,增加對身體的負荷,甚至增加受傷風險

腳掌負重 (Foot Weight-Bearing)
- 有頸/ 脊椎痛的話, 可能會用較多腳掌外側來負重
正確負重:
* 三個位置負重(拇指骨、尾指骨、腳踭)
* 站立/步行的話,建議負重在腳踭較多 >50% [跑步例外]
* 步行/跑步的話,建議負重在拇指骨多於尾指骨
- 每個情況的問題位置都可能不同
*要作出適當評估找出問題 學習腳掌負重會是一個好的切入點

👔Why is my foot being treated when I have neck pain?

Body Compensation
- If we experience upper body pain (especially chronic conditions) such as headaches, neck pain, shoulder pain, or back pain, our body may compensate by using other areas
- For long-term effective treatment addressing the root cause, it is essential to address the lower limbs in addition to the neck.

Interconnection of the Upper and Lower Body
* Using wrong supporting base can decrease the efficiency leads to a decrease in strength and control, increasing the load on body and the risk of injury.
Foot Weight-Bearing
* In the presence of spinal pain, there may be an increased load on the outer side of the foot.
Proper weight-bearing:
* Weight should be distributed among three areas (big toe, little toe, and heel).
* During standing, it is recommended to bear more weight on the heel (>50%) [with the exception of running].
* During walking/running, it is recommended to bear more weight on the big toe than the little toe.

在大多數情況下,我總是優先考慮並信任手法治療技術,多於衝擊波等的機器。它能夠給予作為物理治療師的我即時反饋,從而幫助我更準確地進行治療,並掌握適當的治療強度。

手法治療是獨特且無法被機器或人工智能取代的(至少目前是如此)。

我們必須記住,手法治療仍然是一種被動療法,只能暫時緩解疼痛和症狀。然而,如果正確地使用,它可以在康復過程中提供巨大的幫助,為患者減輕痛苦並增加痊愈的信心,使他們能夠以較少的疼痛繼續進行康復。

🤲🏻Manual Therapy

Manual Therapy is effective in treating spinal, joint, and muscle pain. It is carried out through joint mobilization, trigger point release, and other techniques.

I always prioritize and trust my manual therapy techniques over machines like shockwave therapy in most cases. It provides me, as a physiotherapist, with real-time feedback, which assists me in delivering treatment with better accuracy and intensity.

Manual Therapy is unique and irreplaceable by machines or AI (at least for now).

We must remember that manual therapy is still a passive form of therapy and can only provide temporary relief from pain and symptoms. However, when used correctly, it can greatly enhance the rehabilitation journey, providing relief and confidence to patients as they progress through rehabilitation with less pain.

🪑 Guidelines for Work and Learning Sitting Posture
* Keep the top of the screen approximately 3cm above eye level.
* Maintain an arm distance between your eyes and the screen
* Rest your forearms/wrists on the desk, close to your body. Use them to weight bear for your upper body.
* Lean your body slightly forward.
* Maintain the natural curvature of your spine.
* Keep your thighs slightly pointing downward, with weight supported by your sit bones(Ischial Tuberosity).
* Use your heels to weight bear.

坐太直導致精神&肌肉過度緊張
* 坐太直會激活交感神經 (SNS) -> 導致長期處於精神緊張狀態(fight or flight response)
* 若果未鍛煉好適當既肌肉耐力/張力, 更會增加肌肉疲勞, 引至痛症問題 (尤其頸痛&腰背痛)

將身體輕微前傾令工作學習更有效率
* 激活副交感神經 (PSNS), 有助習中精神、放鬆心情
要注意:
* 前傾時, 也盡量保持脊椎生理孤度 (neutral position)
* 大腿微微向下, 有助減小腰椎壓力

The BEST posture is your NEXT posture
沒有絕對的好姿勢,
但懂得和有能力維持脊椎和其他關節的生理孤度, 會有效減低對身體關節的消耗
* 每30分鐘轉換姿勢
* 使用升降枱/站立絕對是一個好選擇
* 鍛煉好適當既肌肉耐力/張力,幫助維持脊椎生理孤度
身體前傾不代表將脊椎過度向前彎曲

👵🏼Mum tells you to sit up straight (adopt correct sitting posture for work and learning)

Sitting too upright (excessive extension) leads to excessive mental and muscle tension
* Sitting too upright activates the sympathetic nervous system (SNS), leading to prolonged states of mental tension (fight or flight response).
* If the appropriate muscle endurance/tension is not present, it can increase muscle fatigue and result in pain/ symptoms (Especially neck pain & low back pain).

Slightly tilting the body forward for more efficient work and learning
* Activating the parasympathetic nervous system (PSNS) helps with mental focus and relaxation.
Note:
* When tilting forward, try to maintain a neutral spine position.
* Slightly lowering the thighs helps reduce pressure on the lumbar spine.

The BEST posture is your NEXT posture
There is no absolute perfect posture, but understanding and being able to maintain the physiological neutral position of the spine and other joints can effectively reduce wear and tear on the body’s joints.
* Change positions every 30 minutes.
* Using an adjustable desk or standing is a good choice.
* Train appropriate muscle endurance/tension to help maintain the physiological neutral position of the spine.
Leaning forward does not mean excessively bending the lumbar spine forward.

其實好多時都係盆骨後傾
* 只係因為上世紀Dr. Janda提出既理論(如下交叉綜合症等)被普及, 以至盆骨前傾道至腰痛被額外重視

盆骨前傾甚至可以提升運動表現
* 前傾可給身體更多物理運動優勢
* 因為我地隻眼生係前面, 前傾有助我地向前活動; 除非你平日都係向後行

臨床上其實有更多腰痛患者, 係盆骨後傾
* 近年亦有研究指出, 腰痛患者可能有更多後傾角度

我地要知既係:
* 有冇過度前/後傾
* 有冇相關痛症; 影響外觀(社交)
* 有冇相關力量/ 運動上限制
* 點分前/後傾
有影響先要識分/ 處理, 無影響就唔好太在意吧
