Phalangeal fracture series Dec 2024-Jan 2025. PART I

Phalangeal fractures require fixation when there is angulation and rotational deformity. Early fracture fixation with open reduction and internal fixation is advised for early mobilisation to prevent stiffness of the interphalangeal joints, especially the PIPJ

Case 1
- basketball injury. right little finger PP fracture finger overlapping

Case 2
- twist injury of left MF and RF leading to PP fracture of the MF and MP fracture of the RF
- clinical photos showing rotational deformity of the affected digits

Removal of implant +/- extensor tenolysis are recommended 4-6 months after fracture healing, especially in young patients.

Finger masses are always alarming to patients because it can be painful and irritable even when they are very small and subcentimeter.

Any mass that grows bigger in a short period of time warrant medical attention. Even if the mass were benign, they can cause compression to nearby blood vessels or nerves causing damage which can be permanent if left untreated.

This patient had a rapidly growing AVM (arteriovenous malformation) in her index finger. The mass became more painful and grew larger after she had acupuncture.

It is NOT advisable to have invasive treatment in undiagnosed masses.

She was treated uneventfully with excision surgery under a short general anaesthesia.

This patient has multiple painful Swan-neck deformities secondary to connective tissue disease.

Hemi-FDS sling anchored with suture anchor method combined with lateral band translocation were used
for correction.

Post operatively, the patient was fitted with a full-time dorsal blocking splint for PIPJ, allowing immediate post-operative mobilisation.

2 weeks after surgery, the fingers had a good range of options.

This patient presented 4 months after suffering an FPL tendon rupture secondary to distal radius fracture plating (done by another surgeon).

Palmeris longus graft was used for FPL reconstruction. There was heavy scarring in the radial bursa where the distal FPL stump was stuck at; hence, it was opened.

The initial plating was placed very distally at the radius; thus causing attritional rupture to tendon.

Full, strong FPL action was achieved post op.
