胃鏡檢查是怎樣進行的? 我和我的孩子在胃鏡檢查前有什麼準備?

醫生會把一條靈活並附有攝錄鏡頭的喉管(即胃鏡)放進小孩的口腔內,並打入空氣,在鏡頭的即時引導下,便可把胃鏡推進小孩的食道,胃部和一部份的十二指腸作檢查及治療。

很多小孩在胃鏡檢查時需要接受深度鎮靜或全身麻醉,以便檢查能夠順利進行。小孩檢查前約六不能進食或水喝,父母也應已簽署同意書,你也應告訴醫生小孩的詳細醫療記錄和藥物記錄,某些患者如有心臟問題,在胃鏡檢查時可能需要用到抗生素。

甚麼是胃鏡檢查?

這是一個上消化道的檢查,讓醫生能夠直接檢視到口腔,食道,胃部和部份十二指腸的情況,抽樣提取在消化道內組織,檢查是否有幽門螺旋菌感染或其他病理,以便作出即時的上消化道診斷,甚至治療。

診斷-例如:胃炎,上腹疼痛,胃潰瘍,食道裂孔疝氣,覆發性嘔吐,胃酸倒流等。

治療-例如:拿取消化道內異物,上消化道潰瘍止血,食道靜脈曲張治療,放置餵飼喉管等。

認識兒童皮膚疣及治療

疣是皮膚的迅速增生,主要是由於接觸感染了人類乳突病毒(HPV)而造成,可以發生在任何年齡的嬰幼兒身上。 疣並不是癌症, 但可能會帶來疼痛甚至流血,尤其是長在腳底的蹠型疣。

疣通常會在表皮(皮膚表面)形成,其外觀和大小會因為不同的身體部位而不同,並伴有小黑點在中央。一部分的疣可以自行痊癒。 目前大部分的治療方法依賴於直接殺死受病毒感染的皮膚,但會帶來疼痛而且療效也未必理想。

常見方法有:
1. 水楊酸塗液 – 可能需要數個月的療程
2. 冷凍治療 – 以極凍液態氮使受感染皮膚爆裂和結痂,然後疣會隨著結痂一起脫落。
3. 局部麻醉下手術切除

因為個別治療方法可能會帶來疼痛,而且有時需要重複數次才會有效。請家長與醫生討論給予您孩子的最佳治療方案。

什麼是舌繫帶過短及舌繫帶鬆解術

舌繫帶過短是一種先天性發育異常,它限制了舌頭的活動能力,有機會引致進食困難、影響語言能力。

舌繫帶鬆解術是一項簡單手術,可在局部麻醉或全身麻醉下進行,把連接在舌底過短及過緊的一小片舌繫帶切除。術後可顯著改善舌頭的活動能力,亦有助母乳餵哺, 改善言語能力。

什麼是先天性巨結腸症?

先天性巨結腸症是一種罕見而且有潛在機會危及生命的先天性疾病。受影響的新生兒或小孩的腸道某一部分缺乏神經細胞,以使食物/大便到達該段腸道時,未能適當蠕動,造成相關的腸阻塞。

巨結腸症的成因是由於在懷孕期間,嬰兒的神經細胞沿著腸道往肛門方向形成。患有先天性巨結腸症的嬰兒而言,神經細胞在腸道某一個段落停止發育,導致往後至肛門的腸道停止蠕動。

大多數先天性巨結腸症的嬰兒在出生後的數天至數週便會發病。若只有一小段腸道受到影響,受影響的孩子可能會在幾個月,或以至幾年後才逐步出現明顯症狀。

每個孩子的症狀可能會有所不同。新生兒的症狀可能包括有:

-在出生後首48小時內沒有排便
-腹部緩慢腫脹
-嘔吐綠色或棕色的液體

另有些患此症的孩子沒有以上的早期症狀,但可能有以下的病徵:

-慢性便秘(便秘情況會隨年齡增長而變得更糟)
-排出的大便形態細小,水汪汪或帶有血絲
-排便時多屁並以噴射式地排放
-食慾不振
-成長緩慢或延遲

若診斷患有先天性巨結腸症,在身體合適情況下,手術是必須要的。手術包括切除缺乏神經細胞的部份腸道,然後再將健康的腸道連接到肛門縫合。以現在的醫學發展,手術大多以微創形式進行。

Is my child suitable for Magnetic Resonance Imaging (MRI) scan? Can my child cooperate for MRI?

As MRI scan could take up to 30 minutes, although it is painless, sometimes sleeping drugs (sedation) would be required to keep a young child completely motionless for such a long period of time. In very young child, general anaesthesia may sometimes be required. Even a little movement by the child during the MRI scan would significantly affect the quality of the image and therefore resulting in inaccurate reporting.

Regarding the safety of MRI scan, although nowadays, most surgical implants and devices (e.g. cochlear implant and cardiac pacemakers) are MRI compatible, if your child has any foreign objects implanted inside the body, you should bring along supporting document for MRI safety. As MRI involves the use of magnetic field, any metallic implant inside the body would potentially be a contraindication for MRI scan.

We suggest you to consult your doctor if your child is suitable for MRI scan and if any special preparation for your child before the examination.

What is magnetic resonance imaging (MRI)?

It is a painless, non-invasive diagnostic imaging exam that uses magnetic fields and radiofrequency waves to produce accurate 2- and 3-dimensional images of the body’s organ, tissue and bones.

MRI differs from computer tomography (CT) scan and does not use ionizing radiation. MRI and CT scan have different clinical indications and are used in different clinical conditions. Different parts of the body may also be shown up more clearly in a specific radiological images in MRI scan.

MRI examination poses almost no risk to the average patient when appropriate safety guidelines are followed. Apart from potential risk of excessive sedation and malfunctioning of the implanted medical devices, there is a very slight risk of an allergy reaction if contrast material is injected. Therefore, most of children undergoing contrast MRI scan will be best performed in the hospital setting.

What are the risks of Computer Tomography (CT) scan?

1. Allergic reaction to the intravenous contrast used – presenting as hives and skin rashes on the body and could be safely managed by medications.

2. Severe kidney damage – usually only occur in patients with underlying kidney disease receiving intravenous contrast.

3. Ionizing radiation. Similar to conventional X rays, CT scan involves ionizing radiation. In
certain clinical conditions, the benefits of an accurate diagnosis outweigh the risk of exposure to radiation during the exam.

How should I prepare my child for CT scan?

1. To explain to the child in simple terms that the CT scan is not going to hurt, and the machine will not touch the child.

2. Sometimes, a small & soft tube will be put into your child’s vein (heparin block) for the injection of special dye (intravenous contrast) in order to get more clear & accurate images.

3. You should tell the doctor if the child has history of allergy, asthma or kidney disease (see below), as these may increase the risk of allergic reaction to the CT contrast.

4. Dress your child in comfortable clothing that does not contain metal buttons, metal belt buckles or zippers. Metal hair clips and jewelry must also be removed during the CT scan.

5. If sedation(sleeping drug) is required, you may need to fast (no food and water) your child few hours prior to the scheduled CT scan appointment.

What is a computed tomography (CT) scan ?

It is a painless imaging procedure that uses X ray and powerful computer to create detailed, cross-sectional images of your child’s body.

It is an extremely accurate and safe way to quickly assess the internal organs of the child. It is commonly used to diagnose appendicitis; evaluate masses; investigate head injuries, and would rapidly evaluate organ injuries and bleeding in the case of emergency and trauma.

My child has a swelling over his face. My doctor suggest ultrasound scan examination. What if my child won’t stay still for the imaging? When will I know the result of the ultrasound?

A ultrasound scan is a safe and commonly performed test for children. It usually takes about 10 to 20 minutes. Sometimes it is useful to bring the child’s favourite toy, book or electronic device to keep the child occupied and distracted during the examination. In some circumstances, sedation may be required for detailed scanning in a very young child.

Although the ultrasound image is available in real time, it may take some time to prepare a proper ultrasound report in the context of the complete clinical picture. Your child’s doctor will explain to you about the best treatment for your child.
