合作的力量：Pie Pie 從心衰竭到找到希望的旅程
在城大動物醫療中心心臟科，我們的角色是為病人及其家人導航，引領他們在複雜的診斷中認知所有可行的治療方案選擇。這次的個案分享是關於Pie Pie, 一隻勇敢的7歲科頓犬的故事，牠完美地展現了由一個具奉獻精神的家庭、一位細心的家庭醫生以及一支緊密合作的專科團隊之間的協作，如何一起創造奇蹟。
Pie Pie 由蘇利宜動物醫院的優秀團隊轉診至我們的診所，及後診斷出牠患有充血性心衰竭。我們的首要任務是為牠進行量身定制的醫療方案來穩定牠的病情，並提供明確及真誠的預測。
作為牠的心臟科專科醫生，我們的主要角色是規劃整個治療方案：
1. 管理當下：我們細心調整 Pie Pie 的藥物治療，不斷評估牠的狀況，以控制肺水腫並提升生活品質。
2. 規劃未來：從一開始，我們就與非常愛惜牠的主人討論所有長期方案，包括終身藥物管理，以及對部分病例，考慮行經心瓣修復手術的可能性。
3. 建立連結：我們曾與世界一流機構的同事分享複雜病例，能直接促成與皇家獸醫學院（RVC）著名心臟外科團隊的對話，由Dan Brockman教授和Dr. Matteo Rossanese領導。這段合作關係——我們的心臟科專科獸醫João LOUREIRO 醫生曾在英國與Matteo合作，也與Brockman教授分享過多個病例——在Pie Pie的轉診評估和最終改變生命的手術中，扮演了關鍵角色。
團隊合作：
* 家庭：他們堅定的承諾、無私的奉獻與勇氣去追求手術方案，使一切變成可能。
* 家庭醫生（蘇利宜動物醫院：提供關鍵的初步診斷並信任我們，共同管理 Pie Pie 複雜的病況。
* 外科醫生（RVC）：運用他們的世界一流專業技術，完成精細的修復手術。
* 我們的角色（城大動物醫療中心心臟科專科）：在整個過程中扮演著關連的紐帶角色——從最初的導航者、醫療管理者、轉介到外科的橋樑，現在則是作為監督Pie Pie在術後康復的專業團隊。
結果？ 簡直令人驚嘆。
手術後一個月，Pie Pie 已經煥然一新。牠的最新心臟超聲波掃瞄顯示瓣膜完美修復，心臟也顯示出顯著的逆向重塑。牠已從末期心衰恢復過來，進入一個穩定且可控的狀態。
這個案例證明了在現代獸醫醫學中，只要有專業團隊跨診所、甚至跨洲合作，就能創造奇蹟。我們為 Pie Pie 感到驕傲，也由衷感謝牠那了不起的家庭，以及蘇利宜動物醫院和 RVC 的同業們，感謝他們的信任與合作。
At CityU VMC , our role is to be navigators for our patients and their families, guiding them through complex diagnoses and all available options. The story of Pie Pie, a brave 7-year-old Coton De Tulear, perfectly illustrates how collaboration between a dedicated family, a vigilant GP and a connected specialist team can achieve miracles.
Pie Pie was referred to our service by the excellent team at in congestive heart failure. Our initial role was to stabilize him with tailored medical management and provide a clear, honest prognosis.
As his cardiologists, our key role was to map out the entire journey:
Managing the Present: We meticulously managed Pie Pie's medications, constantly re-evaluating him to control his pulmonary edema and give him a good quality of life.
Planning for the Future: From the beginning, we discussed all long-term options with his incredibly dedicated owners. This included lifelong medical management and, for selected cases, the possibility of mitral valve repair surgery.
Making the Connection: Having shared complex cases with colleagues at world-leading institutions, we were able to directly facilitate the conversation with the renowned cardiac surgery team at the Royal Veterinary College (RVC) led by professor Dan Brockman and Dr Matteo Rossanese. This existing relationship, our Specialist in Cardiology, Dr. Joao Loureiro had worked with Matteo in the UK and shared multiple cases with Professor Brockman, was crucial in streamlining Pie Pie's referral for assessment and ultimately, life-changing surgery.
The Team Effort:
The Family: Their unwavering commitment, selflessness, and courage to pursue a surgical solution made everything possible.
The GP (Four Paws AH): Provided the critical first diagnosis and trusted us to co-manage Pie Pie's complex care.
The Surgeons (RVC): Applied their world-class skill to perform the intricate repair.
Our Role ( Cardiology): We acted as the constant thread throughout this journey—the initial navigators, the medical managers, the connectors to surgery, and now the team overseeing his excellent post-operative recovery.
The Result? Nothing short of astounding.
One month post-op, Pie Pie is a transformed dog. His recent echocardiogram shows a beautifully repaired valve, and his heart has undergone significant reverse remodeling. He has moved from end-stage heart failure back to a stable, manageable condition.
This case is a testament to what is possible in modern when dedicated teams work together across clinics and in this case continents. We are so proud of Pie Pie and profoundly grateful to his wonderful family and our colleagues at Four Paws Animal Hospital and the RVC for their trust and partnership.
