睡眠窒息創新偵測

上月提到睡眠窒息的創新手術治療，立即有朋友問怎樣轉介及評估。受睡眠窒息困擾的人實在不少。最近，有不少移動或穿戴裝置都添加偵測睡眠窒息的應用程式，顯見市場的巨大需求。

穿戴裝置中最顯眼是智慧指環(smart ring)。有幾個大的品牌都推出可24小時偵測上20項健康指標的智慧指環，包括心率、呼吸率、血氧度、體溫、睡眠狀況、壓力等，並聲稱可以偵測睡眠窒息。智慧指環的好處是方便，電池續航力較長，有些人覺得睡覺時戴著比戴手錶舒服。根據幾個大品牌公司公布的資料，其裝置對睡眠狀況的偵測有一定的準確度。但在睡眠窒息的偵測方面，各公司都比較小心，只有一兩個品牌公布其智慧指環偵測睡眠窒息準確度的數據，不過都是比較籠統。

穿戴裝置自然包括智慧手錶(smart watch)。領先的品牌離不開蘋果。9月底，蘋果公布其智慧手錶對睡眠窒息的偵測功能，主要用來評估睡眠窒息的風險。從蘋果的數據顯示，其手錶偵測睡眠窒息的敏感性(sensitivity)只有66%左右，特異性(specificity)則達到99%。即是說真正陽性的比率還是比較低，裝置說穿戴者沒有睡眠窒息，並不就是沒有。所以美國FDA只建議作風險評估用，不能做篩查。

我醫院給病人用來偵測並監察睡眠窒息的裝置，是本地創科公司研發的Belun指環，目前很多本地醫療團體、醫生、老人院都有使用。我最近檢視我們自2023年起使用Belun指環病人的數據，發現近380名使用者中，屬於中度或嚴重阻塞性睡眠窒息的患者，大多數是55歲左右的中年人士，以男性居多；中度人士的平均BMI是26至27，而嚴重人士的平均BMI是30至31；有輕度睡眠窒息的，平均BMI也在24至25之間。衛生署對肥胖的定義，是BMI超過25，肥胖始終是導致睡眠窒息的一個很重要的因素。Belun指環在美國取得可推出市場的醫療裝置認證，我們用來偵測、篩查並監察睡眠窒息的進度，對病人自己監控病情有一定的幫助。
