終止自由職業者協議

日期：2025年3月19日

本公告旨在通知公眾，Agile Plus Physiotherapy Limited與陳卓楠先生之間的自由職業者協議已於2025年3月19日即時終止。

陳卓楠先生從未被授權代表或以任何身份代表我司行事。陳先生對外之個人言行及作出任何業務承諾事項，均屬個人行為，不代表我司，本公司亦不為其承擔任何責任。

請注意，任何由陳先生發出的病人治療計劃或收據均不屬於Agile Plus Physiotherapy Limited的責任。如對陳先生的治療計劃有任何查詢，病人應直接聯絡他。

我們感謝陳先生在協議期間所做的貢獻，並祝願他在未來的事業中一切順利。

Public Notice

This notice is to inform the public that the freelance agreement between Agile Plus Physiotherapy Limited and Mr. Chan Cheuk Nam, Benson has been terminated effective immediately as of 19th March 2025.

Mr. Chan has never been authorized to represent or act on behalf of Agile Plus Physiotherapy Limited in any capacity. Mr. Chan's personal words and deeds to the outside world and any business commitments he makes are all personal behaviors and do not represent our company, and our company does not assume any responsibility for matters.

Please note that any patients treatment plans, or receipts issued by Mr. Chan are not the responsibility of Agile Plus Physiotherapy Limited. For any inquiries related to Mr. Chan's treatment plans, patients are advised to refer directly to him.

We appreciate the contributions made by Mr. Chan during the term of the agreement and wish him well in his future endeavors.
