【專欄文章】TOPick — 醫語怡情︰學業壓力大 當心腸易激

每逢測驗考試，甚至面試、比賽等重要場合，有些人總會無故出現肚痛、肚瀉的情況，嚴重時更影響表現。如果感到壓力大或緊張時就會腸胃不適，但身體檢查後卻找不到結構性的病因，就要留意可能是患上腸易激綜合症。

曾有研究顯示，本港逾6%的人口患有腸易激綜合症，而在學生群組中比例更達 10% 至 20%。常言道「腸胃是第二個大腦」，腸道與大腦之間透過「腸腦軸」互相影響，當人承受壓力、焦慮或抑鬱時，腸胃會容易出現不適。臨床觀察發現，每逢開學或考試前後，都有不少學生因相關症狀而求診，當中部分患者的父母同樣受腸易激綜合症困擾。除了來自個人、學業的壓力，家庭環境等因素都會影響情緒，因而引致腸胃症。

曾經有一名16歲的中五男學生，因持續胃脹和腹痛求醫，接受內窺鏡檢查後，排除腸胃結構性的問題，臨床上診斷為腸易激綜合症。醫生處方化胃氣藥和益生菌等藥物以紓緩其病徵，但更需要了解其致病或壓力的源頭。經過詳細問診後得知，該學生的志願是成為一名醫生，為了入讀心儀的醫學院，他對其成績的要求相當高，因學業壓力而出現腸胃不適症狀。筆者作為讀醫過來人，分享了求學時期的個人經驗和抗壓小貼士，並希望讓他明白到，人生隨着不同階段都有不同的苦惱，提醒他要學習情緒管理和調整心態。

腸易激綜合症屬於功能性腸胃障礙，成因未明，可能涉及腸道菌群失衡、蠕動異常，或對痛楚過度敏感。此症大致分為四類：腹瀉型、便秘型、混合型和無法分類型。若連續三個月，每週多於一天出現腹痛、大便頻率或軟硬度改變，而且如廁後症狀有所緩解，在排除其他腸胃疾病後，便可診斷為腸易激綜合症。

治療方面，醫生一般會處方藥物以減輕腹脹、痙攣、腹瀉或便秘等症狀；同時，患者改善生活習慣亦十分重要，例如可按醫生或營養師建議，進行低腹敏（FODMAP）飲食，避免攝取高發酵性短鏈碳水化合物（如豆類、加工食品等），以減輕脹氣和腸胃不適；規律作息、適量運動、充足睡眠，配合適當的壓力管理，也有助穩定病情。

新學年將至，學業固然重要，但健康更是基礎。同學們遇到壓力時，可向家人、朋友或老師傾訴，必要時尋求專業協助，讓身心保持最佳狀態，迎接挑戰。

【秋季防疫🍁減低「雙病毒感染」風險】
秋天將至，呼吸道病毒開始蠢蠢欲動，長者同長期病患者尤其要提高警覺！呼吸道合胞病毒（RSV）嘅傳染力強，初期症狀容易被忽略，而肺炎球菌更可以引致嚴重肺炎或其他併發症。等我哋一齊了解呢兩種病症同相對應嘅疫苗，提防病毒同細菌入侵，守護自己同家人健康！💡

感染RSV 死亡率高過流感❓
RSV 死亡率約11.9%，比流感嘅8.8%更高。近期香港 RSV 感染個案急增三倍，趨勢或會延續，甚至有機會同流感高峰期重疊，增加「雙病毒感染」風險。高風險人士如長者、長期病患者一旦感染，容易惡化成肺炎或心肌炎，所以要特別注意。

孕婦可唔可以接種呼吸道合胞病毒（RSV）疫苗❓
懷孕期間媽媽免疫力會下降，容易受到病毒侵襲。RSV不但會影響媽媽健康，仲有機會影響初生BB。研究顯示，準媽媽喺懷孕32至36周接種RSV疫苗，有效降低BB出生後首六個月感染RSV嘅風險，為BB提供初期保護。

肺炎係香港第二大致命疾病❓
根據2024年數據，肺炎係香港第二大致命疾病，導致超過1.1萬人死亡。 粗死亡率為每10萬人之中，平均每年會有151.5人死亡，排名僅次於惡性腫瘤。其中肺炎鏈球菌係主要元兇之一，20價肺炎球菌疫苗可預防侵入性肺炎球菌疾病，降低併發症及死亡風險。

20價肺炎球菌疫苗（PCV20）涵蓋20種血清型❓
20價肺炎球菌疫苗涵蓋20種常見血清型，適合18歲以上高風險人士，接種一劑就可以提供持久防護。建議長者、曾有侵入性肺炎球菌病史、患有新陳代謝疾病（包括糖尿病、肥胖）及免疫力較低嘅人士接種，有效降低感染風險。

【Get vaccinated to reduce the risk of "Dual Virus Infections"】
As fall draws near, respiratory viruses are beginning to re-emerg, and the elderly and those with chronic illnesses need to stay vigilant! Respiratory Syncytial Virus (RSV) is extremely contagious, and its initial symptoms can be easily missed, whereas pneumococcus can result in severe pneumonia or other complications. Let’s learn about these two viruses and their corresponding vaccines to safeguard ourselves and our families against viral and bacterial threats! 💡

RSV vs Influenza Virus Infection ❓
Mortality was found to be higher in the RSV group (11.9%) than in the influenza group (8.8%). Recently, there has been a threefold increase in RSV cases in Hong Kong, and this trend could potentially coincide with the peak of influenza in the near future, increasing the chance of "dual virus infection". High-risk groups, such as the elderly and individuals with chronic health conditions, are more susceptible to severe complications like pneumonia or myocarditis following infection, so extra caution is needed.

Can pregnant women get the RSV vaccine❓
During pregnancy, a mother’s immunity may decrease, increasing her vulnerability to viruses. RSV not only poses a risk to the mother’s health but can also affect newborns. Studies indicate that pregnant women who receive the RSV vaccine between 32 and 36 weeks of pregnancy can effectively lower the risk of their baby contracting RSV in the first six months after birth, thereby providing early protection for the infant.

Is pneumonia the second leading cause of death in Hong Kong❓
According to 2024 data, pneumonia ranks as the second leading cause of death in Hong Kong, resulting in over 11,000 deaths. The crude mortality rate is 151.5 deaths per 100,000 individuals annually, second only to malignant tumours. Pneumococcus is one of the main culprits, and the pneumococcal 20-valent vaccine (PCV20) helps prevent invasive pneumococcal diseases, reducing the risks of complications and mortality.

Does the Pneumococcal 20-valent vaccine (PCV20) cover 20 serotypes❓
The pneumococcal 20-valent vaccine covers 20 common serotypes and is recommended for high-risk individuals aged 18 and above. A single dose provides long-lasting protection. It is particularly advised for the elderly, those with a history of invasive pneumococcal disease, metabolic conditions such as diabetes or obesity, and immunocompromised individuals to mitigate the risk of infections.
