【專欄文章】晴報 — 醫語怡情：男士性健康不容忽視

男士隨年齡增長可能會出現各種性健康問題，有研究指出，本港每兩名40至70歲男士中，就有一人曾受過不同程度勃起功能障礙的困擾。勃起功能障礙不僅影響生理健康，更關乎心理健康、伴侶關係及生活質素，然而，不少男士或因難以啟齒而延誤就醫。

勃起功能障礙的成因涵蓋生理與心理層面，生理因素包括糖尿病、高血壓、心血管問題等慢性疾病；心理因素如焦慮、抑鬱或伴侶關係緊張亦可能導致性功能障礙，部分患者更同時受多個因素所影響。

曾有患者可能怕尷尬諱疾忌醫，自行嘗試其他偏方或產品，結果因未見成效而決定求醫。其實，除了親身到診所求醫，現時視像會診相當普及，有需要的男士可安在家中接受診症，減少隱私顧慮及心理負擔。視像的方式有助病人更放鬆地接受診療，同時可鼓勵他們正視性健康問題。

在視像會診過程中，醫生主要透過詳細詢問病人的健康狀況，包括慢性病史、曾否接受過手術、生活習慣及體能等，便可初步作出適切的判斷。對於大部分的患者，醫生可按臨床判斷直接處方藥物，病人一般會即日獲安排藥物送遞。如涉及其他疾病，醫生會建議病人接受進一步驗血或體格檢查等作跟進。

治療方面，現時以口服藥物為主，有關藥物可放鬆血管、改善陰莖血流，有助病人在性刺激下恢復勃起功能。不同藥物的生效時間與藥效維持時間各異，維持時間由數小時至十多小時不等。醫生會因應病人狀況及生活需求，建議最合適選擇。若病人正服用降血壓或心臟藥物等，則須事前諮詢醫生意見。如藥物效果不理想，病人可以考慮注射或手術治療。醫生亦會視乎病人情況及需要，安排心理治療、性輔導轉介等。

早前有一位四十多歲的大腸癌病人，術後因神經受影響而致出現勃起功能障礙，醫生進行評後，為病人處方口服藥物，並轉介其接受性治療，最終病人成功克服性障礙，與伴侶重拾親密關係。

男士遇上性健康問題不必默默承受，建議及早正視並尋求專業治療。

【港怡參與厨尊飯菜預備活動　以行動關懷社群】

港怡醫院在可持續發展之路一直實踐IHH醫療集團「關懷﹒至善至遠」（Care. For Good.）的精神，積極回饋社會，並致力為社會大眾帶來正面的影響。

今天，港怡的管理層及員工參與由新加坡駐香港總領事館主辦的「SG60 Support Initiative for Project Dignity (Hong Kong) 」，親身到訪「厨尊」的廚房，穿上圍裙與手套協助製作餐盒，並送到低收入家庭、長者及無家者等有需要人士手中，為本地社群送上關懷。

社會企業 源自新加坡，致力透過技能培訓、職場實習和就業配對，幫助有需要人士特別是長者及殘疾人士。旗下的「厨尊」在香港營運一間富有新加坡小販中心特色的社企餐廳，並積極推行社區外展活動，如免費膳食派送「請你食飯」計劃，為社會上有需要的一群送上一份溫飽。

Driven by IHH Healthcare’s aspiration to “Care. For Good.”, Gleneagles gives back and makes a positive impact to our community.

As part of the SG60 Support Initiative for Project Dignity (Hong Kong) hosted by the Consulate General of the Republic of Singapore in Hong Kong, our senior leaders and staff put on their aprons and gloves to help at the Dignity Kitchen to prepare and distribute meal boxes to the underserved including low-income families, the elderly and the homeless.

Founded in Singapore with the mission of social inclusion, the social enterprise empowers disadvantaged individuals especially the elderly and the disabled through training, placement programme and career opportunities. Apart from a hawker-style food court, its Hong Kong branch, Dignity Kitchen, also runs community outreach programs, including meal distribution to underserved groups.

【專欄文章】晴報 — 紅腫痕癢的蕁麻疹

蕁麻疹（俗稱「風癩」）是一種常見的皮膚過敏反應，患者皮膚會突然出現紅腫、痕癢的腫塊。曾有病人因全身出疹而求醫，問診後發現皮疹在其每天趕上班出汗後發作，為減少出汗，病人需每天乘搭的士上班，甚至避免運動，日常生活受到影響。醫生診斷為出汗誘發蕁麻疹，病人接受治療後成功控制病情，重新恢復正常生活與運動習慣。

蕁麻疹可分為急性和慢性，大多數情況下會在24小時內自行消退，但也可能反覆發作。病發少於六星期屬急性，成因多數由過敏或感染引起，例如花粉、塵蟎或食物敏感、病毒或細菌感染等；慢性蕁麻疹一般維持超過六周，或與紅斑狼瘡、甲狀腺病等免疫系統疾病有關，但亦可能沒有特定原因。流汗、壓力、情緒起伏等均可為誘因，若本身屬過敏體質，如患有鼻敏感、濕疹、哮喘等，風險亦較高。

蕁麻疹可發生在任何年齡，約有40%的人一生中至少患一次蕁麻疹。蕁麻疹的出現主要是因為皮膚組織內的白血球受到刺激而釋放組織胺，引起紅腫與瘙癢症狀。紅疹可分佈於全身不同位置的皮膚上，若出現嘴唇或面部腫脹以致呼吸困難，則可能是蕁麻疹引發血管性水腫，應立即求醫。如皮疹伴隨痛感，或需進一步檢查以排除血管炎等其他自身免疫疾病。

醫生一般會根據患者的臨床症狀及病史作出診斷，如有懷疑，例如患者是女性且對陽光敏感，會進一步安排驗血，以排除紅斑狼瘡症。輕微個案不一定需要使用藥物，可嘗試冰敷患處以緩解癢感，並建議穿著寬鬆衣物、避免搔抓患處以防病情惡化，部分患者可能會因反覆搔抓導致蕁麻疹演變為濕疹。如已知蕁麻疹因過敏引致，應盡量避免接觸懷疑致敏源，情緒放鬆亦有助穩定病情。

此外，醫生會因應患者情況考慮採用藥物治療，通常會處方口服抗組織胺類藥物，以紓緩敏感徵狀。若情況嚴重者，可考慮入院接受抗免疫球蛋白E的生物製劑注射以快速改善病情。

蕁麻疹發作時，患者除了出現皮膚紅疹及痕癢外，生活質素亦會下降，有部分患者更因此容易出現焦慮或抑鬱等情緒問題。事實上，多數蕁麻疹患者只要按照醫生的建議或接受適當治療，病情通常能得到有效控制。若皮膚經常無故出疹，建議及早求醫，以找出過敏根源，對症下藥。
