【專欄文章】TOPick — 醫語怡情︰突發腹痛或因黃體囊腫破裂

偶爾有女病人因突然出現下腹劇痛而求診，伴隨噁心、嘔吐及發燒等症狀，誤以為是急性闌尾炎，經檢查後發現原來是卵巢内的黃體囊腫破裂出血。症狀輕微的患者僅需要服用止痛藥及休息便能痊癒，嚴重者則可能需要接受手術清除血塊。

黃體囊腫破裂與月經的週期息息相關，常見於17至44歲、正值生育年齡的女性。每次排卵後，剩餘的濾泡會形成黃體，並分泌黃體酮，是穩定子宮內膜、幫助受精卵著床的關鍵荷爾蒙。若該排卵周期內沒有懷孕，正常情況下，黃體會在約一星期後自然萎縮並消失，導致黃體酮水平下降、子宮內膜剝落並排出，因而出現月經。然而，若黃體未有萎縮，內部液體持續增加，便會形成黃體囊腫，嚴重甚至可增至大於 3 厘米，會有破裂風險，尤其是當身體不慎受到外力撞擊，例如激烈性行為或劇烈運動，黃體囊腫有機會因扭轉、破裂而造成内出血。當血液流出到腹腔時，腹膜受刺激，會引致下腹劇痛等症狀。

有醫學文獻指出，女性右側卵巢出現黃體囊腫破裂的機會，較左側卵巢高；原因可能是左側卵巢周圍受乙狀結腸保護，以及右側卵巢排卵的機率較高。筆者偶爾也會遇到黃體囊腫破裂個案，據臨床觀察，當中每年只有幾宗嚴重至需要接受手術治療。如懷疑是黃體囊腫破裂，在排除懷孕的可能性後，醫生一般會安排超聲波、電腦掃瞄等檢查作評估及診斷。

即使出現黃體囊腫，多數會於三個月內被身體吸收，大部分個案毋須手術處理。在此期間，醫生會為病人處方止痛藥，並建議多加休息、避免劇烈運動及性行為，以減少併發症風險。如囊腫破裂並出血情況嚴重，病人或需接受手術，以止血及清除在腹膜的血塊。

早前有一名三十多歲的病人，因腹部劇痛及血壓偏低，到醫院求診。醫生安排驗孕的結果呈陽性，而超聲波檢查結果亦顯示病人已懷孕 10 週，同時右側卵巢有一個 5 厘米的黃體囊腫已破裂及出血。由於情況緊急，醫生立即進行腹腔鏡手術以控制內出血，並需要切除黃體囊腫。黃體酮在懷孕初期對胚胎發育十分重要，醫生術後為病人處方了兩星期口服黃體酮作補充，病人隨後孕期未有因這次黃體囊腫破裂而受影響。

曾經歷過黃體囊腫破裂的女士，再次發生的機率與一般女性相若，實際發生與否仍較難預測。對於本身有服用避孕藥的女性，因避孕藥可抑制排卵，從而減少黃體囊腫的形成，可降低復發風險。黃體囊腫破裂雖然不常見，但症狀有時與其他急症相似，容易被忽略。如出現突發下腹劇痛，應及早求醫檢查，以釐清病因並接受合適治療。

During the Chinese New Year holidays, ’ 24-hour Outpatient and Emergency Department will remain in service to safeguard your health day and night. Gleneagles is launching a carfare rebate programme from midnight on 16 February to 11:59pm on 22 February 2026. Within the designated period, patients arriving at our 24-hour Outpatient and Emergency Department by taxi or Uber can enjoy a carfare rebate of up to HK$200.

Carfare rebate programme:
-Taxi/Uber fare rebate applies to a one-way trip to Gleneagles Hospital Hong Kong only.
-The fare will be deducted from the total bill amount at a maximum rebate of HK$200 per consultation.
-Don’t forget to get the taxi receipt or download the Uber e-receipt from the app or email, as you will need to present and submit the receipt upon payment.

【區域協作　攜手推動靜脈病學發展】

【Regional collaboration for advancing excellence in venous care】
