ACL REHAB: Hurry up and WAIT!

🔬The research is crystal clear on this, returning to playing sports earlier than 9 months after ACL surgery significantly increases your risk of suffering a re-tear!

‼️In fact, many researchers still recommend waiting until a full year after surgery to return to playing sports for most athletes below a professional level.

👉We need to remember what an ACL Reconstruction is. A surgeon is taking a portion of a tendon, usually from your hamstring or patella and replacing your ACL with it.

🙌Fortunately, we aren't relying on the tendon graft itself to immediately hold up as a functioning ACL.

💯A successful ACL reconstruction relies on your body's ability to incorporate the tendon graft into your knee and change the properties of that tendon into those of a ligament.

🏋️‍♂️ How the disc deals with axial force

“Imagine pressing with your hand on a water balloon, which is sitting on a table. You can see that the pressure causes the balloon to splay out laterally, and in all directions equally.

The nucleus of the disc holds a lot of water for the same reason: under pressure, it splays out laterally, and in all directions equally.

When the nucleus makes its water-balloon motion and splays out at all sides, the annulus (a tyre-like restraint around the nucleus) accepts that outward force.

Now, axial forces coming down the spine are directed not just down but out, into the waiting arms of the annular rings.

So, you might have heard that the nucleus is a shock absorber, but as you can see that is not quite right. It’s better to call the nucleus a shock distributor; or, even better, a force distributor.”

🔎The post images are from a case report that documents the resolution of a lumbar disc herniation without surgery and demonstrates that pain can return when the apparent disc ‘injury’ is no longer present. Pain is complex!

🧠The MRIs shown here are from a 32 year old male who presented to the hospital with severe back pain in two different occasions. On October 2012, the patient did not have any neurological deficits, so physical therapy and a short course of oral steroids were ordered and his pain resolved.

👉In October 2013, another MRI was ordered when the same patient once again entered the hospital with severe back pain. This time around, his disc herniation was no longer visible.

⁉️What does this teach us⁉️

1️⃣Pain does not necessarily indicate tissue damage. In both situations documented above, the patient had low back pain. At the time of the first episode, a potential tissue insult could be identified. This was not true at the time of the second episode of back pain.

2️⃣Disc herniations can resolve with conservative care and time. The details of this individual’s rehab were not given, but it is important to know that relatively simple programs that include movements like walking, general low back mobility drills and low load resistance training exercises help the majority of people. A healthy dose of patience (up to 4-6 months) is important too.
