Efektivitas Edukasi Terapi Insulin terhadap Pengetahuan dan Perbaikan Glikemik Pasien Diabetes Melitus

Diabetes Self Management Education (DSME) merupakan salah satu strategi penting dalam penatalaksanaan Diabetes Melitus (DM). Manajemen terapi insulin merupakan peran penting dari tenaga kesehatan untuk meningkatkan pengetahuan dan tercapainya kontrol glikemik yang lebih baik. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian edukasi manajemen terapi insulin tehadap peningkatan pengetahuan dan perbaikan kontrol glikemik (penurunan HbA1c) pada pasien DM tipe 2 yang menggunakan insulin. Penelitian menggunakan desain RCT (randomized control trial) dengan pengambilan sampel secara systematic random sampling. Subjek penelitian dibagi dalam kelompok kontrol yang tidak mendapat intervensi dan kelompok uji yang mendapat intervensi edukasi manajemen terapi insulin oleh farmasis selama 1 bulan. Data yang diperoleh dianalisis menggunakan statistik uji t-test. Hasil uji paired t-test pada masing-masing kelompok uji dan kontrol menunjukkan bahwa ada perbedaan yang bermakna antara pengetahuan sebelum dan sesudah edukasi (p=0,001 dan p=0,011) dan tidak ada perbedaan antara HbA1c sebelum dan sesudah edukasi (p=0.274 dan p=0,166). Hasil uji independent t-test menunjukkan bahwa edukasi manajemen terapi insulin efektif terhadap peningkatan pengetahuan (p=0,034), tetapi tidak efektif terhadap perbaikan kontrol glikemik (p=0,463). Diabetes Support Groups Improve Patient’s Compliance and Control Blood Glucose Levels

Providing information is not enough to improve diabetic patient’s compliance and achieve goals of therapy. Patient’s good awareness as well as emotional and social supports from family and community may play an important role to improve their compliance and clinical outcomes. Therefore, diabetes support groups were developed and each support group consisted of two pharmacists, two nurses, diabetic patients and their family members. A total of 70 type 2 diabetic patient’s were enrolled and randomized into support group 1 and support group 2. Patients in the group 1 received information leaflets only, while patients in the group 2 received pharmacist counselling and information leaflets at each meeting. Patient’s awareness of diabetes and compliance with medications were assessed by a short questionnaire at baseline and final follow-up. Blood glucose and cholesterol levels were also evaluated in both groups. At the end of study, the overall patient’s awareness and compliance improved by 61.5%. The random and fasting blood glucose levels decreased over than 30% in the group 2 and around 14% in the group 1. This study reveals that collaboration between health care professionals and community in the diabetes support group might help diabetic patients to increase their knowledge and compliance with the diabetes therapy as well as glycaemic control. Effect of Attapulgite on the Oral Bioavailability of Ciprofloxacin

Adsorption nature of attapulgite may inhibit the absorption of ciprofloxacin after oral administration. As a result, this drug-drug interaction may reduce ciprofloxacin bioavailability. This study was aimed to determine the effect of attapulgite on the bioavailability of a single orally-administered ciprofloxacin. Six New Zealand white rabbits received each of the following treatments in a randomized, three-way crossover sequence, separated by a 7-day washout period: (i) ciprofloxacin (23 mg/kgBW) alone; (ii) ciprofloxacin (23 mg/kgBW) given simultaneously with attapulgite (28 mg/kgBW); (iii) ciprofloxacin (23 mg/kgBW) given 2 hours after attapulgite (28 mg/kgBW). Blood samples (1 mL) were collected from the marginal ear vein up to 240 minutes postdose. The plasma concentrations of ciprofloxacin were determined by a validated high-performance liquid chromatography method. Study of Pre and Post Supplementation Green Tea Extracts to MDA Levels

High intensity exercise (HI) induces oxidative stress. Running for 2400 meters is one of the simple exercise form which can be counted as HI. During HI, increase of oxygen consumption happened in muscle cell resulted in an increase of oxidant level. The aim of the study was to measure the effects of Indonesian green tea supplementation, before and after HI to the level of lipid peroxidation (TBARSMalonyldialdehyde (MDA)). The subjects of study were 42 male students from Faculty of Medicine, Universitas Padjadjaran (FMUP) and Faculty of Physical Exercise, Indonesia University of Education (FPOK-UPI). The subjects were divided into trained and untrained subject. Subjects were divided into groups using Astrand Harvard step test. The purpose of this test was measuring the level of VO2 max. Blood samples for MDA level were collected at 3 hours before and after high intensity aerobic exercise. Oxidative stress was induced by 2400 meters run. The result showed that green tea supplementation was effective to decrease MDA plasma level especially after exercise. Hubungan Obesitas dan Hipertrigliseridemia dengan Risiko Perlemakan Hati pada Pasien di Makassar

Perlemakan hati terjadi akibat lipolisis berlebihan dari jaringan lemak dan peningkatan suplai asam lemak bebas ke hati. Derajat keparahan perlemakan hati berkorelasi dengan peningkatan berat badan. Penelitian ini bertujuan untuk menemukan hubungan antara obesitas, trigliserida, obesitas dan hipertrigliseridemia terhadap risiko terjadinya perlemakan hati. Penelitian cross-sectional menggunakan data retrospektif yang diambil dari pasien check up di Laboratorium Klinik Prodia Makassar selama tahun 2006. Diagnosis perlemakan hati melalui pemeriksaan ultrasonografi, obesitas ditentukan dari indeks massa tubuh, dan trigliserida diukur dengan metode enzimatik kolorimetri. Dari 204 pasien check up, 67 pasien (32,8%) mengalami perlemakan hati. Pada pasien dengan perlemakan hati sebanyak 95,5% mengalami obesitas, 67,2% hipertrigliseridemia, dan 32,8% trigliserida normal. Potensi Interaksi Obat Resep Pasien Geriatri: Studi Retrospektif pada Apotek di Bandung

Usia geriatri merupakan kelompok usia yang rentan terhadap masalah-masalah yang terkait dengan penggunaan obat, salah satunya adalah kejadian interaksi obat-obat. Dalam penelitian ini dilakukan studi untuk mengetahui interaksi potensial obat-obat. Data diproses melalui www.drugs.comdatabase. Evaluasi ini memaparkan prevalensi dan mengklasifikasikan jenis interaksi potensial berdasarkan level interaksi dan spesialisasi medik. Dari total 29.839 resep dari tujuh apotek di kota Bandung diperoleh 334 lembar resep geriatri (1,12%). Dari resep geriatri tersebut, tedapat 4 lembar resep (1,20%) dengan jumlah 1 R/ yang artinya pada resep ini tidak berpotensi untuk terjadi interaksi. Sedangkan jumlah R/ pada lembar resep yang mengandung lebih dari 1 R/ adalah 1.136 dengan rata-rata jumlah R/ pada setiap lembar resep adalah 3,40. Sebanyak 131 lembar resep terdapat interaksi potensial obat-obat sebesar 39,22%. Total interaksi potensial yang terjadi adalah 210 interaksi. Interaksi potensial moderate adalah sebanyak 187 (89,05%) sedangkan severe sebanyak 23 (10,95%). Kejadian potensi interaksi moderate dan severe pada kelompok spesialisasi medik umum adalah sebanyak 85,00%, penyakit dalam 8,40%, kardiologi 2,30%, THT 2,30%, syaraf 0,76% dan gigi 0,76%. Peran Farmasis Klinis pada Pasien Gagal Ginjal Tahap Akhir dengan Hemodialisis

Pasien gagal ginjal tahap akhir dengan hemodialisis merupakan populasi pasien dengan kondisi multipatologis dan multiterapi yang laju morbiditas serta mortalitasnya terus meningkat. Tujuan penelitian ini adalah untuk mengetahui peran farmasis klinis pada manajemen perkembangan penyakit gagal ginjal tahap akhir yang menggunakan terapi hemodialisis di Indonesia. Penelitian ini menggunakan metode kajian sistematis melalui penelusuran database penelitian. Penelitian yang terlibat terdiri atas 2 penelitian dengan metodologi ekperimental dan 8 metodologi deskriptif. Jenis peran farmasis klinis yang dilakukan adalah review catatan terapi (90%), evaluasi terapi (70%) dan implementasi terapi (20%). Penelitian yang ada telah memberikan gambaran mengenai peran farmasis klinis pada populasi pasien gagal ginjal dengan hemodialisis. Gambaran tersebut masih terbatas pada aktivitas pengkajian atau assessment. Hal ini karena penelitian yang berfokus pada peran farmasis klinis secara umum di Indonesia juga terbatas jumlahnya. Correlation between Low Grade Inflammation with Endothelial Progenitor Cells Surface (EPCs) Marker in Hypertension

Patients with hypertension have been shown to express reduced number and functional capacity of endothelial progenitor cells (EPCs). Recent study reveals that EPCs contain an important capability to maintain endothelial integrity and vascular homeostasis. Therefore, enhancement of EPCs could be benefit for individuals with cardiovascular diseases. We investigated the correlation between low grade inflammation marker (hsCRP) with EPC surface marker (CD34 total) in hypertension. This was an observational study with cross sectional design conducted in 51 nonhypertenive and 60 hypertensive subjects. hsCRP (as marker of inflammation) was measured by immunochemiluminometric method. CD34 total (as marker of EPC) was measured by flow cytometry method. hsCRP concentration was higher in hypertensive subjects compare to nonhypertensive subjects (mean±SD, 2.709±2.50 vs 2.476±2.438 mg/L; P>0.05). Pharmacists’ Interventions in A Paediatric Haematology-Oncology Pharmacy: Do They Matter to Minimise Medication Misadventure?

Paediatric patients with cancer are a high-risk patient population for medication misadventures. This study aimed to document and evaluate the role of pharmacists’ interventions during dispensing-related activities in minimising the occurrence of medication misadventure in haematology-oncology patients. The primary investigator observed and documented all clinical interventions during dispensing-related activities performed by clinical pharmacists in a haematology-oncology pharmacy during 33-day. A total of 359 interventions were performed for 1028 patients. The rates of intervention were 20.04 per 100 medication orders and 34.92 per 100 patients. Provision of drug information was the most common interventions constituting more than three quarters of all interventions. According to therapeutic groups, cytotoxic antineoplastics made up more than half of all interventions. Of all interventions, 22 involved recommendations leading to changes in patients’ treatment (active interventions), and all recommendations were accepted. The top three medication errors were due to inappropriate dosing, labelling error, and unfulfilled indication. Clinical pharmacists’ intervention during dispensing in a paediatric haematology-oncology pharmacy improved medication safety and patient care by minimising the incidence of medication misadventures. Polifarmasi dan Interaksi Obat Pasien Usia Lanjut Rawat Jalan dengan Penyakit Metabolik

Penyakit metabolik merupakan penyakit yang berkaitan dengan peningkatan usia seperti hipertensi, diabetes melitus, dislipidemia, dan obesitas. Penyakit-penyakit tersebut ditangani dengan terapi obat yang sifatnya polifarmasi sehingga dapat menyebabkan risiko terjadi interaksi obat-obat. Penelitian ini dilakukan secara restropektif dengan menggunakan rekam medis pasien usia lanjut dengan penyakit metabolik yang di rawat jalan di Rumah Sakit Umum Pusat Haji Adam Malik Medan. Sebanyak 328 sampel yang termasuk ke dalam kriteria inklusi diperoleh data bahwa jumlah interaksi obat-obat yang terjadi cukup tinggi sebesar 78,96%. Pola mekanisme yang terbanyak adalah farmakokinetik (63,6%) dengan tingkat keparahan yang terbanyak adalah moderat (69,8%). Penelitian ini menunjukan adanya korelasi antara jumlah interaksi dengan jumlah obat (r= 0,728; p= 0,0001), dan jumlah interaksi dengan jumlah diagnosis (r= 0,264; p= 0,0001).
