Harmony™ Prenatal Test
DNA from the fetus circulates in the mother’s blood. Cell free DNA (cfDNA) results from the natural breakdown of fetal cells (presumed to be mostly placental) and clears from the maternal system within hours of giving birth.

During a pregnancy, cell free fetal DNA can be tested to give the most accurate screening approach in estimating the risk of fetus having a common chromosome condition sometimes called a trisomy. This occurs when there are three copies of a particular chromosome instead of the expected two. The test looks to detect the following trisomies:

Trisomy 21 is the most common trisomy, at the time of birth. Also called Down syndrome, it is ssociated with moderate to severe intellectual disabilities and may also lead to digestive disease, congenital heart defects and other malformations.

Trisomy 18 (Edwards syndrome) and Trisomy 13 (Patau syndrome) are associated with a high rate of miscarriage. These babies are born with severe brain abnormalities and often have congenital heart defects as well as other birth defects. Most affected individuals die before or soon after birth, and very few survive beyond the first year of life.

S*x chromosome conditions. The s*x chromosomes (X and Y) determine whether we are male or female. X and Y chromosome conditions occur when there is a missing, extra, or incomplete copy of one of the s*x chromosomes. The Harmony with X, Y test can assess risk for ###, XYY, XXYY, XXY (Klinefelter Syndrome), and a missing X chromosome in a girl (Turner Syndrome). There is significant variability in the severity of these conditions, but most individuals have mild, if any, physical or behavioural features.
If the mother is interested in having this optional testing, she should talk with her healthcare provider.

INFERTILITY INVESTIGATIONS
The major causes of fertility problems are:
• Failure to ovulate
• S***m insufficiency, eg. quantity and/or quality
• Damage to the fallopian tubes preventing the egg and s***m coming together
• Endometriosis
• Unexplained infertility - for nearly one third of people there seems to be no obvious cause for the infertility as everything appears to be ‘normal’

Infertility Blood Tests
Follicular Bloods: These routine blood tests are usually obtained between days two to four of your menstrual cycle (where day one is the first day of bleeding). They may also be obtained at any time if the periods are either infrequent or completely absent.
The following blood test results are used to investigate whether ovulation is occurring (production of an egg) each month and also to help determine which fertility treatments to offer.
FSH (Follicle Stimulating Hormone) - Follicle-stimulating hormone (FSH) helps control a woman’s menstrual cycle and the production of eggs. FSH test is done on the second to fourth day of the menstrual cycle and can be used to evaluate the egg supply in the ovaries. FSH stimulates the ovaries to produce follicles, which are fluid filled sacs, which grow, develop and release an egg.
LH (Luteinising Hormone) - In women, luteinising hormone (LH) is linked to ovarian hormone production and egg maturation. An LH test is used to measure a woman’s ovarian reserve (egg supply).
Progesterone - Progesterone is a female hormone produced by the ovaries during ovulation. It causes the endometrial lining of the uterus to get thicker, making it receptive for a fertilised egg. In a regular 28 day cycle, the day 21 progesterone result is used to determine if ovulation is occurring.
Prolactin and Thyroid Function – Prolactin is a hormone produced by the pituitary gland (in the base of the brain) and may be requested if your periods are irregular or absent.
Abnormalities of the thyroid gland may also result in a failure to ovulate regularly.
Rubella (German measles) antibody levels: It is important to look for evidence of immunity, as rubella infections in pregnancy can lead to various abnormalities of thedeveloping baby. If you are not immune then vaccination can be arranged.

Semen Testing
Semen samples should be produced by ma********on into a container provided by the clinic.
For optimum results we recommend three (maximum five) days abstinence from s*x and ma********on before sample production.

Ultrasound Scans
The trans-va**nal (internal) Ultrasound scan is one of a number of investigations performed when assessing your fertility. The scan can give extremely useful information about your womb (uterus) and ovaries that will help make decisions about your fertility management.
A probe is inserted into the va**na and images of the womb (uterus) and ovaries can be seen on the scan monitor.

Hysterosalpingogram (HSG)
This is an x-ray examination of the fallopian tubes to ensure the tubes are not blocked. It needs to be performed during the 10 days following the start of a period.
A negative Chlamydia swab is needed before this test can be done.
HyCoSy
The HyCoSy (Hysterosalpingo-Contrast Sonography) test is an ultrasound investigation used to check that your fallopian tubes are open. At the same time it allows us to also look at the uterus (womb) and ovaries.

Ovarian Reserve Testing
Anti Mullerian Hormone (AMH)
This blood test measures the hormone excreted by the granulosa cell which surrounds the follicles in the ovaries. Follicles are the fluid filled sacs on the ovaries which contain the egg which is released at ovulation. Results are reported in numerical value and fit into ranges; optimal, satisfactory, low and undetectable fertility.
Antral Follicle Scan (AFC)
This ultrasound examination looks at the ovaries and the number of tiny follicles that are present in each o***y. This examination is ideally performed from day 5 - 10 of the menstrual cycle but can be done at any time. Research has shown that as the ovaries age, the number of these tiny follicles declines.
The result of these tests advises of a female patient’s fertility potential and what treatment is best suited to the number of follicles remaining in the ovaries.
Laparoscopy/Hysteroscopy
The role of surgical investigation (laparoscopy +/- hysteroscopy) for female infertility is controversial, but many pathologies contribute to infertility (endometriosis, polyps, submucosal fibroids, hydrosalpinges) and treatment can significantly improve outcomes. Investigative surgery is most clearly indicated for those women with symptoms.
The management of couples with infertility depends on the cause, whether one or both partners affected and the severity of the problem.
